Inspection conducted with Rich Gulyas, SWN Construction Foreman and Seth Tratthen, Regulatory Analyst. The well permit that has been issued for this pad is the Karmazin 3H (115-21835). All perimeter BMP’s in place to include red lined additions. Pad is 95% complete. No violations noted.

The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure and the valve going to the production tank was closed. There was 562psi on the production casing and 402psi on the production line. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 2:25p.m.

The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (13x9) annuli were below snow and ice, unable to check for gas. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2779174 Primary Facility 2018-09-25 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. The cellar was full of water and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2816479 Primary Facility 2018-12-17 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. All 5 cross culvert rock sumps along access road are present and functional. No E&S issues identified. Good vegetative stabilization present. No violations present.
