2526693 Primary Facility 2016-10-11 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 9:40a.m., I met with company man Robert Gray, Falcon Rig 23 was on site and drilling the vertical section of the well to kick off. I took a walk around the location and there were no obvious concerns noticed.

Depth at time of inspection was 4218'. Projected TD for the top hole section is 5918'. I left the site at 10:40a.m.

2531798 Primary Facility 2016-10-17 Routine/Complete Inspection No Violations Noted Falcon Drilling Rig #23 is currently on the location in the process of drilling the Gaylord 4H well. SWN submitted a drilling resumption After Break of 30 days or more notification to the Department on 10-11-16. The comments in the notice indicate that Falcon 23 will skid 20 feet from the Gaylord 2H to Gaylord 4H on 10-12-16 at 3:30 PM. SWN submitted a casing notification to the Department on 10/11/16. The notice indicated that Falcon 23 will be setting 664' 13-3/8" H-40 48# STC casing and cementing back to surface with Schlumberger services on 10/13/16 at 6:00 AM. I met with Tony Anthony, site company man, consultant of SWN. The current drilling depth is (3,931) feet. Mr. Anthony stated the following. The intermediate casing on the 4H was set at (1,644) feet. The 4H will be drilled to (5,748) feet which is the kickoff point. The well will be loaded with oil based mud once it is drilled to kickoff point (100) feet above kickoff point. The 1H and 2H wells have been drilled to kickoff point. After th

I met with company men Ken Bush and Mike Tornes. US Well Services was on site to stimulate the 1H, 2H, 4H, 5H, and 6H wells. They were fracturing stage 4 out of 31 stages on the 4H. The 1H and 2H were completed with 25 stages on the 1H and 23 stages on the 2H. The 5H and 6H haven’t been started yet. There will be 57 stages on the 5H and 27 stages on the 6H. The water was being pumped to the site from the Zeffer production water impoundment. I took a walk around the location and there were no obvious concerns noticed. I left the site at 9:40a.m.

2585450 Primary Facility 2017-04-21 Routine/Complete Inspection No Violations Noted Inspection conducted on 4/18/17. Complete circuit of entire site found no violations. Observation of the stimulation operation occurring on the pad identified no spills. No violations observed.
2621157 Primary Facility 2017-08-01 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:35a.m.

The well was recently completed and is in production. The well tag was on the well and correct. There was containment around the well and three sealed roll off containers were on the containment, the containment appeared to be in good condition. The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 559psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 12:30p.m.

2660346 Primary Facility 2017-10-25 Routine/Complete Inspection No Violations Noted The Department was on site, to conduct a Routine Complete Inspection, in response to the receipt of a Well Site Restoration report. The site appeared to be reflective of the Restoration Plan therefore the Department acknowledges the Restoration.

The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (13x9) annuli were below the water level and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2832950 Primary Facility 2019-01-29 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was ice 1.5' below grade in the cellar and was unable to check for gas. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2855230 Primary Facility 2019-03-12 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. As the photo's show, the site is snow covered. Overall the site appears to have good stabilization. No E&S issues identified. No violations noted.
