2600921 Primary Facility 2017-06-05 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 9:10a.m. for a new spud inspection.

Minichi Contracting Group drilled the holes and ran the conductor pipe with a dual rotary rig on the 1H and 2H wells. I spoke with welder Mylo Welter of Minichi who was welding caps on the conductor pipe, and a backhoe was digging out around the conductors to set the cellars. I checked the drillers log book to get the depth of the conductor pipe, the 1H and 2H were both drilled and set to 70'. The drill cuttings were put into a sealed role off container and hauled to the Keystone Landfill. I took a walk around the location and there were no obvious concerns noticed. I left the site at 9:25a.m.

2608708 Primary Facility 2017-06-27 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 9:00a.m.

I met with company man Robert Gray. WSG Rig #1 was rigged down and crews were rigging down and hauling equipment off the location. I took a walk around the location and there were no obvious concerns noticed. Depth at time of inspection was 1657'. I left the site at 10:15a.m.

2629656 Primary Facility 2017-08-22 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:30p.m.

I met with company man Harry Stevens. Halliburton was on site to stimulate the 1H and 2H wells. They completed 2 stages and was starting stage 3 out 34 stages on the 1H. They completed 6 stages out of 28 stages on the 2H. Stage 6 pressured up at the end of the stage, so they were unable to pump the plug and perf down for stage 7. They will finish the 1H then go back to the 2H. The water was being pumped to the site from the Zeffer production water impoundment and Odell fresh water impoundment. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:20p.m.

2640125 Primary Facility 2017-09-21 Incident- Response to Accident or Event Violation(s) Noted Eric Rooney (DEP) and I traveled to the NR 65 Leonard Well Site to conduct an Incident Inspection in response to a self-reported release. We met with Mike Deming (SWN) on site for the inspection. Mr. Deming explained that on 9/19/17 SWN began to remove the well pad containment. Then on 9/20/17 around 5-6 pm they continued working to find that after the sun dried out the surface of the site, they noticed features indicating a brine release such as crystallization and dark staining that wasn't drying out. Mike Deming was notified at that point and he mobilized an excavator and sealed roll off containers. Being that the volume couldn't be quantified, SWN will be following Act 2.

Seth Tratthen (SWN) and Dillon Otten (SWN) returned at 0630 on 9/21/17 and conducted conductivity screenings at location points a-v and found a range in conductivity of 200uS -3.12mS. 2661907 Primary Facility 2017-11-16 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed. It was shut in at time of inspection. The well tag was in place and correct. The cellar was set up with a corrugated pipe around the well for inspection access and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed.
