2726955 Primary Facility 2018-05-07 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the SWEPI LP - Kinnan Unit 845 located in Middlebury Twp, Tioga Co. on 5/7/2018 @1115 hours. The weather was 55° cloudy, winds north at 8 mph.

At the time of this inspection the H&P rig was being constructed onsite. The primary rig containment was already in place. Trucks carrying pieces of the rig were observed driving back and forth across containment. The strad tanks were staged on containment. No SWEPI personnel was encountered during the inspection. No erosion or sedimentation was observed.

Four new well permits were issued 3/22/18 117-21974 21H 117-21975 23H 117-21976 25H 117-21977 28H

The 1V 117-20691 was SPUD 11/16/2010 and is in RIS

No violations are noted at this time. I departed the site at 1200 hours.

2728522 Primary Facility 2018-05-10 Drilling/Alteration No Violations Noted At time of inspection rig crew was still setting up location and finish rigging up. No know issues with any of the wells on the pad.
2735599 Primary Facility 2018-05-30 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the SWEPI LP - Kinnan Unit 845 well pad located in Middlebury Twp., Tioga Co on 5/30/18 @1230 hours. The weather is as follows:

Winds from the SSE 13 mph Humidity: 55% Pressure: 30.07 in ¿ UV Index: 4 Cloud Cover: 87% Ceiling: 9100 ft Dew Point: 64° F Visibility: 9 mi

Heath McAfee, SWEPI OSR was present onsite. The Ensign 162 rig is currently onsite. SWEPI is in the process of drilling the 23H well. The access road, E&S controls, containment and tank pad were observed during this inspection. The pad was dry. No puddles or staining was observed. No evidence of erosion or sedimentation was observed. The tank farm is being utilized for personnel trailers and equipment. Two diesel tanks are stored on containment but are empty at this time. No violations are noted at this time. I departed the site at 1420.

2772554 Primary Facility 2018-09-07 Routine/Complete Inspection No Violations Noted The DEP was onsite for a tour of SWEPI LP's fracking operations at the Kinnan 845 well pad located in Middlebury Twp, Tioga Co. on 9/7/2018 @ 1000 hours. Present for the inspection onsite was myself, DEP WQS Env Trainees Korey Starkey, Mike Jerballa, Sara Metz. SWEPI Personnel onsite Pat Bernethy, Env. Coordinator; Phil Tillitt, Completions OSR; and John Rowell, Ops. Field Lead.

SWEPI is currently in the process of fracking the 21H, 25H and 28H wells. SWEPI plans to be onsite until approximately October 1st. The access road, well pad, tank pad, primary containment and secondary containment surrounding the rolloff tanks was examined during the inspection.

We departed the site at 1230 hours. No violations are cited at this time.
