Inspection performed as a result of a waiver review due to the proximity of the well-site to an Unnamed Tributary of Little Mahoning Creek, which is a HQ-CWF. Due to the HQ-CWF status of the tributary, there are additional E&S concerns that should be addressed in the E&S plan before the Waiver is approved. The Following deficiencies were found with the E&S plan: 1- The Department requests that the operator rock the entire access road with clean durable rock, to limit sedimentation from entering the stream. 2- To further limit sedimentation, the operator should rock the well pad at any location that will be affected by vehicle movement. Any other areas that are affected by earth disturbance activities should be seeded and mulched whenever possible.
