Shared access road adequately rocked, noted "steep" contour of access road. Well staked not yet drilled. It appears that operator has installed all bmp's as outlined in plan, although it is noted that brush/grub line shall be installed inside compost sock (or filter fence)-dept recommends that operator continue to closely monitor and maintain all bmp's in order to prevent a possible pollution event to waters of the commonwealth. A diversion ditch appears to be installed along access road and behind pit with ultimate dissipation at down gradient bmp's-- at this time ditch has not yet been rock lined.ecommends rock be added to the ditch in order to reduce accelerated erosion and sedimentation concern. One below grade (unlined) pit located onsite, operator plans to encapsulate drill cuttings. Pit located outside of previously hydric soils--does not (visually) appear pit is located within seasonal high groundwater table.
2122359 Primary Facility 2012-12-07 Administrative/File Review In Compliance with Policy admin inspection result of OG-71 request for approval to solidify drill cuttings fom below the casing seat in an onsite pit. As per the attached request, operator plans to remove free liquid fraction- haul to approved treatment facility, add and mix "bulk" cement into cuttings ensuring not to compromise structurally integrity of liner. Subsequently liner will be carefully folded over top "solidified" pit and back filled according to regulations.

Rig currently onsite, although not actively drilling. Appears that operator has implemented bmp's as outlined in ES plan. If operator continues to closley monitor and maintain well site and access road HQ stream shall be reasonable protected from a potentail prollution event. One below grade lined pit located (>100ft from stream) onsite with fluids and cuttings present with adequate freeboard. Site and road appear rocked, no accelerated erosion or sedimentation issues noted during this inspection. Although department reccomends that order to prevent a possible pollution event, operator add additional rock to access road, culvert outlets and diversion ditches both along access road and well site.

Drill pit previously solidified and encapsulated on 2/22/13. During today’s inspection, one above grade pit located onsite with fluids, freeboard adequate. Site appears muddy due to recent heavy precipitation events, recommended to onsite Rep Bill Hoover that operator add additional rock to well site and access road and/or temporarily stabilize site asap with seed and mulch as well as continue to closely monitor and maintain bmp's in order to prevent a possible pollution event. Areas surrounding the well site and access road were snow covered, unable to accurately determine stabilization status, although no accelerated erosion or sedimentation issues noted.
