Access road entrace and roadway adequately rocked. Based on ES plan, operator did not install compost sock (instead hay reinforced filter fence) and culvert outlets have not yet been rocked; although seeded and mulched vegetation capable of resisiting erosion has not yet been established. Hay reinforced filter fence is not an ABACT bmp utilized in HQ watershed, although in this case appears operator is utilized a well vegetated filter strip in addition to reinforced filter fence. Department reccomends operator follow ES plan as outlined as well as the utlization of compost sock in HQ washersheds. One below grade, lined drill pit located onsite. Pit contains adequate freeboard. Operator plans to encapsulation drill cuttings onsite, according to soil characteristics SHGWT is > 80in. No accelerated erosion or sedimenation issues noted during this inspection.

Above mentioned inspection returned to Department by USPS as 'not deliverable as addressed'. Inspection re-mailed to Home, PA address. Department recommends operator update records to reflect current address. Violation for failure to install permit number on a completed well, failure to submit well site restoration report, and failure to submit a well record and completion report remain outstanding. Operator has until 12-18-17 to respond to the NOV issued on 11-1-17.

UPDATE 2-6-18: Site visit. At time of inspection Department unable to locate well tag. Department contacted operator rep Gordon Phillips by phone. Mr. Phillips stated well tag will be placed on well head by 2-10-18. A follow up inspection will be conducted at a later date to ensure well tag is present. UPDATE 2-13-18: Operator rep Gordon Phillips contacted Department by phone and stated well tag is on well head. Operator appears to be in compliance at this time. Previously issued violations under inspection records listed above for failure to submit restoration report, failure to submit well record, failure to submit completion report, and failure to place well tag on a completed well have been corrected and closed.
