2104615 | Primary Facility | 2012-10-03 | Routine/Partial Inspection | No Violations Noted | I conducted a pre-operational inspection of the site. I conducted a partial inspection because active blasting and construction activity were occurring during the inspection. Perimeter BMPs have been installed. |

2133596 | Primary Facility | 2013-01-25 | Drilling/Alteration | No Violations Noted | No one on site. 2 half rounds (empty) and backhoe on site. Conductor and rathole are compoleted, but cellar is not dug out. No well tag. |

2264416 | Primary Facility | 2014-04-09 | Routine/Complete Inspection | No Violations Noted | Flowback procedures were in progress for the Reel Unit Macialek 1 Reel 1H well. The Macialek Unit Macialek 1 1H well is in production. Flowback procedures commenced on 4/7/14 according to Citrus. The flowback process has not returned any gas to the surface as of yet. An in line heater, separation unit and gas buster tank staged on secondary containment are utilized for the flowback process. Flowback water is transferred to frac tanks in the frac tank battery on location next to the Pad berm. According to Citrus, most of the frac tanks are empty. Upon filling the tanks with flowback water, the tanks will be transported to the P&G (5) Well Pad for reuse as well stimulation procedures are on-going for the PREVOST UNIT P&G 5 PREVOST 1H and RUARK UNIT P&G 5 RUARK 4H wells. There were some areas of compromised integrity in the frac tank battery secondary containment system. |

2744982 | Primary Facility | 2018-06-13 | Routine/Complete Inspection | No Violations Noted | I arrived at the site at 13:43. The weather conditions are cloudy skies with light winds and temperatures around 70 degrees Fahrenheit. There was no activity or personnel at the site. The Macialek 1H and Macialek- Reel 1H wells are in the production phase of operations. A service rig performing similar workover operations on the Ruark South Pad, has recently moved off this location according to Reilly and BKV personnel during the Department's inspection at the BKV Ruark South Pad on 6/13/18, inspection ID# 2743281. The secondary containment dedicated for work-over operations remains in place. A frac tank most likely containing production fluid generated from BKV wells and used during the work-over process, also remains in place. I observed a petroleum sheen on containment near the Macialek Reel 1H well. I used a blue Hanna HI 9831 Soil Test EC & Temperature meter, and white Hanna HI 9831 Groline Soil Test meter to conduct field screening of specific conductance (conductivity) of water on secondary |
2754154 | Primary Facility | 2018-07-10 | Follow-up Inspection | No Violations Noted | Matt Shope, DEP O&G WQS and I arrived at the site at 12:08 to conduct a follow-up inspection. The weather conditions are mostly sunny skies, warm and humid, with light winds and temperatures in the mid 80's. There was no activity or personnel at the site during the inspection. The most recent inspection conducted by the Department was on 6/13/18. During the 6/13/18 inspection, I discovered a petroleum sheen on the secondary containment near the wellheads, along with elevated conductivity readings in water in containment near the wellheads and containment unit for a frac tank. For additional details refer to the Department's 6-13-18 inspection report, inspection ID # 2744324. The frac tank and secondary containment have been removed from the site. Conductivity levels were elevated in water in the production fluid tanks containment unit and in the Macialek-Reel 1H well cellar. See photos. The Department left the site at 12:59. |
