2484597 | Primary Facility | 2016-01-04 | Routine/Partial Inspection | No Violations Noted | On 1/4/16, I performed a routine partial inspection at the Gulick site and made the following observations:
•Landslides/slumps along the access road have not been remediated but have do not seem to have gotten worse. No violation. •A small drilling rig and associated equipment was on site at the time of inspection. |
2485986 | Primary Facility | 2016-01-22 | Incident- Response to Accident or Event | In Compliance with Policy | On 1/22/16, I performed an incident inspection at the Gulick site in response to a spill report received that morning and made the following observations:
•Per the operator a hose disconnected/was not connected properly while transferring reuse water between from a truck to a storage tank allowing roughly 50 gallons to spill out of containment and onto ice on the pad. Of the 50 gallons spilled roughly half was reported to have been immediately removed via vacuum truck before the ice was melted. The remaining amount was to be removed once the pad thawed (pad surface was hard frozen at the time of inspection). |

This request was approved as a minor modification. •The vegetation cut slope north of the site entrance continues to be sparse with a number of small slumps present. No violation. •Two erosion channels along the access road have been repaired with rock linings. No violation. |
