1833363 Primary Facility 2009-10-06 Routine/Complete Inspection No Violations Noted Drilling was ongoing at the time of the inspection. Below grade pit located onsite and it had more than the required two feet of lined freeboard. No access road associated with this site as it is located adjacent to a parking area. Noted no accelerated erosion & sedimenation associated with the site, however mud and sediment were being transported off-site onto the pavement of the parking area and park road. This was due to the deep muddy conditions of the well pad. The operator should continue to monitor the traffic leaving the site to insure that mud & sediment is not being transported out onto SR 1033 and to insure the erosion controls at the site continue to minimize accelerated erosion & sedimentation.

1850008 Primary Facility 2009-12-15 Routine/Complete Inspection No Violations Noted Drilling had been completed at the time of the inspection. Well appeared to be equipped for & in production at the time of the inspection. Site had been regraded, and the below grade pit that had been onsite had been backfilled. Observed no accelerated erosion & sedimentation with respect to the access road or well-site. The Department recommends that the operator continue to monitor the erosion controls through the winter months to insure that they continue to function properly. The operator might also consider mulching the site to provide a layer on top of the soil to minimize accelerated erosion & sedimentation until weather conditions permit seeding of the site for final restoration.
1976472 Primary Facility 2011-05-27 Site Restoration No Violations Noted Drilling was completed on 10/07/09. Well appeared to be equipped for production. Observed no accelerated erosion or sedimentation in relation to the access road or well-site. Vegetative cover met the 70% uniform coverage criteria. Operator should remove the silt fencing and stakes left onsite as a temporary erosion control.
