2618329 Primary Facility 2017-07-19 Routine/Complete Inspection No Violations Noted On July 19, 2017, the Department conducted a routine inspection at the SWN Production Company, LLC, GU-I-Pad in Stevens Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for SWN.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The slopes of the pad were well vegetated and no erosion issues were observed. A chainlink fence was observed around the perimeter of the pad. The slopes of the pad and surrounding area is utilized as a pasture by the landowner. An infiltration berm was observed to be recently installed north and west of the well pad. The berm was observed to be installed as per the approved ESCGP. The berm was temporarily stabilized with mulch. Compost filter sock was installed down gradient of the infiltration berm.

Seven wells were in production and one well was approved for inactive status. The wells were labeled and no bubbling was observed in the well cellars. Two productio

2740733 Primary Facility 2018-05-21 Routine/Complete Inspection No Violations Noted I arrived at the site at 13:06. The weather conditions are mostly sunny skies with a light breeze and temperatures in the low 70's. There was no activity or personnel at the site. Completions activities have been completed in a relatively recent time frame. All of the wells on the pad are in production status. It appears as if a new production fluid storage tank has been installed in a containment structure, with the containment structure appearing to be recently spray painted with a poly urea substance. There is a green in color vacuum box tank staged on containment in between the production fluid storage tank and Diaz container. The tank is most likely staging sand/solid materials generated from flowback or production fluid from the wells. The containment did not appear to be adequately secured. I discovered trash scattered in a few areas of the site. The Ball-Meyer 4H well did not have a tag displaying the permit number. See photos. I departed from the site at 14:14.
