BOPs last tested on 1/31/15. Walked around the site and rig and all appeared to be in good shape. (Note: the Cooley 1H was drilled 07/24/11. The area around the cellar was snow covered). 2355144 Primary Facility 2015-03-10 Routine/Complete Inspection No Violations Noted Saxon Rig #172 is at the site. Jack Burns of Talisman Energy is the Company Man on-site. Saxon 172 has drilled the Cooley 5H well to a total depth of (14,240) feet, with the production casing cemented last night according to Mr. Burns. Mr. Burns stated the Cooley 2H, 3H and 4H have been drilled to total depth, the Cooley 1H which was in production, is currently shut in and Saxon 172 is scheduled to move off this location to Talisman's nearby Upham Well Pad on 3/11/15. While in the process of surveying the site, I observed a rainbow colored petroleum sheen in water on the pad surface in the Northwest corner of the Pad at N- 41 50' 14"; W- 76 14' 19". Mr. Burns informed me that the sheen will be immediately removed from the water via on-site vacuum unit.

2598972 Primary Facility 2017-05-25 Routine/Complete Inspection No Violations Noted On May 25, 2017, at 1130 hrs., the Department conducted a routine inspection at the Repsol Oil & Gas USA Inc., Cooley P 05-004 well site in Orwell Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Repsol.

The site sign, E & S plan and the weekly inspection logs were on site and available for review. The slopes of the pad were well vegetated. PCSM has been addressed at the well site.

Five wells were in production during the inspection. All of the wells were labeled and no bubbling was observed in the well cellars. Two production tanks and one frac tank were observed in secondary containment. Puddles on the well pad surface were low in specific conductivity. No staining was observed on the well pad.

The well pad has not been reduced, but PCSM has been addressed. A two year restoration extension has been approved by the Department for the Cooley P 05-004 well site.

No violations were observed. The Department left the

2705435 Primary Facility 2018-03-12 Routine/Complete Inspection No Violations Noted On March 12, 2018, the Department conducted a routine inspection at the Repsol Oil and Gas USA LLC, Cooley P 05-004 in Orwell Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for Repsol.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site was partially snow covered but appeared to be well vegetated. No erosion issues were observed. The well pad has not been reduced in size, but PCSM has been addressed. Two infiltration basins have been installed at the site.

Five wells were in production during the inspection. The wells were tagged and no bubbling was observed in the well cellars. Two production tanks and five GPUs were also on site. No conductivity readings were taken because all puddles were frozen. No staining was observed on the well pad surface.

No violations were observed during the inspection.

2774327 Primary Facility 2018-09-11 Site Restoration No Violations Noted The DEP conducted a RESTORATION inspection of the Repsol Cooley (05-004) P Pad in Orwell Twp, Bradford County on 9/11/2018 at 1145 hours. The inspection was completed in response to a Notice of Termination received by the DEP. The weather was 66 degrees, overcast and 85% humidity.

There are 5 producing and 1 plugged well onsite with two production tanks. All PCSM practices appear to be installed and working properly. The site appears to be well vegetated, stabilized, and no temporary E&S BMPs were observed. A post drilling well site restoration report was accepted by the DEP.

This Notice of Termination is recommended for approval. No violations are noted at this time.

I departed the site at 1225.
