I approved of an alternative waste management practice (OG-71) for temporary storage an on-site pretreatment system of top-hole water, drilling fluid, hydro-fracturing flow back fluid and production fluid on 9-12-12. Darren Bishop and Frank Kelly of SEPCO were on-site during the inspection. SEPCO was in the process of preparing for flowback procedures. I was informed by Darren Bishop that a test run of filtration has been conducted thus far. Several light towers were observed to be housed on the surface of the ground of the Well Pad and there were a few areas of compromised liner integrity noted during the inspection.

2584487 Primary Facility 2017-04-17 Routine/Complete Inspection No Violations Noted On April 17, 2017, at 1200 hrs., the Department conducted a routine inspection at the SWN Production Company LLC, Fields (GU-1-Pad) Location in Herrick Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Two SWN employees were on site performing maintenance work on the GPUs.

The E&S plan, site sign, and weekly inspection logs were on site and available for review. No erosion issues were observed. The slopes of the well pad were stabilized with rock. PCSM infiltration berms were stabilized with mulch and erosion control matting.

Five wells were in production during the inspection. All wells were labeled properly. The well cellars were filled in with gravel material. One production tank and five GPUs were observed on secondary containment. Liquid within both secondary containments was low in specific conductivity. Puddles on the pad surface also test low in specific conductivity. No staining was observed on the well pad surface.

There are no active

The eSubmission Post Drilling Site Restoration Report was submitted to the DEP on 8/28/2018. Based on this inspection report the Post Drilling Restoration report was accepted on 8/29/2018.
