2583113 | Primary Facility | 2017-04-12 | Follow-up Inspection | Outstanding Violations - Viols Req'd | This was a follow up inspection to the February 15, 2017 response letter regarding the NOV dealing with the oil that is leaking from this well. It looked like the only work that had been done was the placing of hay bales on the ground. There were six hay bales down and they were not placed in areas to keep the oil from continuing to enter a nearby wetland. The amount of oil that has continued to leak from the well has probably increased to around 25 to 30 gal. Most of it is still contained to the area around the well but evidence of it being some 50' away is now present. Mr. Bortnick was contacted by telephone and told that his efforts are not satisfactory and that work has to be done at this site to contain and clean-up the oil as soon as he can. He was told that he should probably hire somebody if he could not do the work. |

2597863 | Primary Facility | 2017-05-19 | Follow-up Inspection | Outstanding Violations - Viols Req'd | Stopped to do a follow up inspection on this well after the 2nd NOV letter. No new activity has been performed. There is now approximately 4 - 8 gallons of new and weathered crude oil on the ground around the well. The hay used to soak up the oil from previous clean-up attempt is still down. There appears to be no fluid getting into the plastic tote that the line from the well is hooked into. The violations are still present. |
2598970 | Primary Facility | 2017-05-30 | Follow-up Inspection | Outstanding Violations - Viols Req'd | Follow up inspection. More clean up work has been done since the last inspection. Most of the oil has been removed (maybe 95 to 98% of it). Some residual oil is still present in the rocks and soils around the well head and on the ground 25' from the well. The areas near the wetland should have the soil scraped up. It appears that only sorbent material was used to clean up the oil with no soils removed. No other work has been done to the well. There is no fluid in the tote. The spill can not be called cleaned up until the residual oil and soils have been removed. |

2627016 | Primary Facility | 2017-07-31 | Follow-up Inspection | Violation(s) Noted | Met Russ Wetherell (Complete Energy Services) and Pete Lower (contractor) out at this site. They had two roll-off containers delivered to this site. They started work by scraping the top 2-4 inches of old oil stained soil below the well site. They dug down into the fragmites to get to a boundry end point. We agreed on a boundry and a depth of soil depth removal in the worst areas of the stained soils. They also dug out around the well casing to uncover the annulus and reveal the 8 5/8" casing to a depth of about 3 feet. Work will continue today and tomorrow on this clean up. |
2627081 | Primary Facility | 2017-08-08 | Follow-up Inspection | Violation(s) Noted | Stopped and inspected this site and found the site work was completed with all the contaminated (oil stained soils) removed. The casing was still open around the 8 5/8" casing. The total area was smoothed over and was mulched. The roll offs were removed. Russ Wetherell (CES) and Pete Lower were present. |
2627091 | Primary Facility | 2017-08-17 | Follow-up Inspection | Violation(s) Noted | Stopped and inspected this site with Cory Deniker (DEP). The site work still looks good with vegetation now starting to grow up through the mulch in all areas where the soils were removed and replaced. The area around the casing was still dug up and open around the 8 5/8" casing. Nothing leaking at this time. Annulus is still plumbed to a plastic tote. No fluids in the tote. The well has not produced since they violations were first noted. |
2631625 | Primary Facility | 2017-08-31 | Follow-up Inspection | Outstanding Violations - Viols Req'd | Well has electric pumpjack and 250 gal tote at wellhead. Annulus is plumbed to tote. Recent work has been done to cleanup at wellhead, separator and tank battery. Vegetation becoming established below wellhead. Small pit/ dike dug around wellhead while source of previously noted leak is determined. Contaminated soil has been removed from near the separator and in the tank battery by Complete Energy Services. Operator is replacing tanks. Existing violations for abandoned well and defective casing. No new violations. |
2658295 | Primary Facility | 2017-11-07 | Follow-up Inspection | Outstanding Violations - Viols Req'd | Recent work done at tank battery to reconstruct dike to contain tank and separator (Photo # 1). Old tank on site. "New" tank is one of the old ones flipped over and repaired. Contaminated soil has been removed from near the separator and in the tank battery by Complete Energy Services. Existing violations for abandoned well and defective casing. No new violations. |
2715173 | Primary Facility | 2018-04-05 | Follow-up Inspection | Outstanding Violations - Viols Req'd | Electric pumpjack on wellhead. Tote at near wellhead. Recent cleanup work done at and downslope of wellhead; contaminated soil removed, seeded and mulched. Well is not producing. Outstanding violation for abandoned well. |
