2609045 Primary Facility 2017-06-27 Incident- Response to Accident or Event Pending Inspection conducted in response to incident report received from Vavco, LLC. I arrived on site at 10:20 hrs. Mr. Calvin Bristol of Bristol Excavating was present.

As reported, the contractor noticed a hydrocarbon odor while working which turned out to be from an encapsulated pit from previous oil and gas activities. At the time of the report, the contractor estimated that approximately 0.5 gallon of oil was encountered. A roll-off dumpster was delivered to the site for storage and transport of material excavated during removal of the pit.

At the time of my inspection, the contractor was in the process of excavating the pit. The material excavated had already filled the roll-off dumpster on site, and the contractor was preparing to bring another for the remaining material. The filled dumpster looked to be about 20 cubic yards in size, and was lined and covered to protect and contain the contents.

The cuttings pit had been fully excavated, and 2 roll-offs full of material were on-site. A trench had been dug from the pit extending approximately 25-30' to the south; the purpose of the trench was unclear. The exposure of the drill pit was referred to the Environmental Cleanups Program at the DEP. In accordance with 58 C.S. 3216(d), the site must be restored within 9 months after plugging, which was done on 11/10/2016; this includes revegetation.

The initial inspection was conducted on June 27, 2017 (Insp. ID. No. 2609045).

During site reclamation activities, the contractor unearthed an area where a hydrocarbon odor was detected. A petroleum sheen was observed on standing water within the excavated area. A roll-off dumpster was ordered by the operator and the contractor began excavating the area, unearthing the encapsulated pit, exposing its contents. As observed during the June 27 inspection, water was flowing from within the encapsulated pit and accumulating in the excavation. A petroleum odor was evident.

Per the Well Site Restoration Report WSRR filed on September 16, 2011, the material disposed of in the pit included drill cuttings, cement returns, and frac sand. The WSRR indicates that the pit was 60 feet long, 40 feet wide, and 10 feet deep, and the line was folde
