2596724 Primary Facility 2017-05-22 Routine/Complete Inspection No Violations Noted On May 22, 2017, at 1225 hrs., the Department conducted a routine inspection at the Repsol Oil & Gas LLC USA, Peck Hill Farm 05-180 well site. The Department was represented by Andrew Hobbins, Water Quality Specialist. Nobody was present for Repsol.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site was well vegetated and no erosion issues were observed. PCSM has been addressed at the site with two bioretention basins installed at the northern and southern corners of the pad.

One well was in production during the inspection. The well was labeled and no bubbling was observed in the well cellar. Two production tanks and one GPU were observed in secondary containment. Liquid within the containments was low in specific conductivity. Puddles on the pad were also low in specific conductivity. No staining was observed on the well pad surface.

The well pad has not been reduced in size, but PCSM has been addressed at the site. A two year restoratio

2792758 Primary Facility 2018-10-22 Site Restoration No Violations Noted NOT approval.

I took electrical conductivity readings of pooled liquid with an Oakton PCSTestr 35. A puddle near the production tanks returned a 0.07mS/cm today. 5 cellars (2H-6H) are in a row on the pad. JLP
