2549599 Primary Facility 2017-01-03 Drilling/Alteration Pending Frac Tech International

Inspected site with Department Oil & Gas Inspector Supervisor, Oil & Gas Inspector and Water Quality Specialist following fire on location at approximately 3:50 p.m. on Sunday 1/1/2017. Frac Tech International was in the process of fracturing the Marcellus Formation. At the time of the fire there were 7 stages to go of 68 stages on this well. Each stage is approximately 135 ft. long. Operator representatives gave possible cause of fire as hydraulic line rupture by a motor. Operator is still investigating cause of fire and will follow up with Department.

2550837 Primary Facility 2017-01-01 Drilling/Alteration No Violations Noted The department was called out by PEMA for a fire on the Rice Papa Bear pad at approximately 16:03 on January 1st, 2017. Upon arrival at well site at approximately 17:50 the majority of the fire had been extinguished using firefighting foam. Foam was still being applied and used to extinguish truck tires that had not yet been fully extinguished. The department conducted an inspection of fracing equipment, wellheads and the containment on the pad. The fire appeared to be limited to the area around the frac pumping truck units and associated equipment. Several pump trucks were severely damaged with other equipment requiring further inspection to determine damage and safe operation ability.
2551823 Primary Facility 2017-01-03 Incident- Response to Accident or Event Violation(s) Noted On January 3, 2017, I inspected the Papa Bear well site accompanied by OGI Supervisor Scott Sabocheck, OGI Eric Peterson, and OGI John Rushin as well as a number of Rice Energy representatives in response to a fire and subsequent loss of brine and other fluids to the pad that occurred on 1/1/17. Observations from the inspection are below.

•At the stream crossing on the site access road substantial amounts of gravel and mud had been sprayed over the CFS and into the immediate area of the stream. The issue was communicated to site personnel at the time of inspection and immediate actions were taken to remediate. No violation at this time. •The fire and associated efforts to extinguish it resulted in currently unknown amounts of brine, oil, firefighting chemicals, and other pollutants reaching the surface of the pad via numerous holes burned in the liner. This constitutes a violation of 25 Pa. Code § 78.57(a) and 25 Pa. Code § 78.54 in that production fluids were spilled to the ground. •At the time of inspe

2554492 Primary Facility 2017-01-13 Drilling/Alteration No Violations Noted Frac Tech International

Resumed fracking operations on 1/12/17 after fire at site. Working on last 4 stages.

2560624 Primary Facility 2017-01-03 Drilling/Alteration No Violations Noted Inspect location for progress in cleanup and preparation for final frac work to be completed after service company inspects iron and equipment exposed to fire. In process. NOVIO
2561005 Primary Facility 2017-01-11 Drilling/Alteration No Violations Noted Inspect location for progress in cleanup and preparation for final frac. Company safety representative indicates fracing operations to start tomorrow. All pressure equipment has been replaced and/or repaired as needed for safe operation. 2614227 Primary Facility 2017-04-26 Follow-up Inspection No Violations Noted On 4/26/17 a follow up inspection was conducted of the Papa Bear Well Site in response to an incident (CTS ID: 324312) that occurred on 1/1/17. The following was observed:

>The site appeared to be in generally good condition with no visible residual impacts from the fire.

>Two graveled areas were found at the site's stream crossing directly adjacent to the delineated wetlands there. Wetland vegetation was observed encroaching on disturbed and graveled areas. Minor modifications should be added to site plans. No violation.

>Secondary containment around equipment on site was intact and adequately maintained.

>The site appeared to be in generally good condition with no visible residual impacts from the fire.

2616619 Primary Facility 2017-07-07 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected On 7/7/17 an INCDT inspection of the Papa Bear site was conducted in response to an operator reported spill of 25-30 gallons of produced water on 7/6/17 (CTS ID: 328060). The following was observed:

>>The pull offs near the stream crossing on the site access road noted during previous inspections had been removed and vegetated as reported by the operator.

>>The operator reported that while servicing well equipment an undrained section of pipe leaked 25-30 gallons of production water to the pad. Excavation of the impacted materials had been completed at the time of inspection with no further indication of contamination. This constitutes a violation of 25 Pa. Code 78.54 and 25 Pa. Code 78.57(a) in that the operator failed to contain production fluids.

>>Servicing of site equipment was ongoing at the time of inspection.

2747937 Primary Facility 2018-07-02 Routine/Complete Inspection No Violations Noted This well has been completed and is producing. Wells and site appear to be well maintained.
2776258 Primary Facility 2018-05-03 Compliance Evaluation No Violations Noted On 5/3/18 a complaint evaluation inspection was conducted at the Papa Bear Well Site in response to CTS ID 328060. No evidence of residual contamination found. Complaint closed.
2780721 Primary Facility 2018-09-27 Follow-up Inspection No Violations Noted On 9/27/18 a routine partial inspection of the Papa Bear Well Site was conducted in response to a bulk earth moving notice related to a major modification at the site.

>>Earth moving was underway at the time of inspection with no noted issues.

>>Major modification plans as well as original plans are included in the site photos. Plans include conversion of site sediment traps into wet ponds as well as reducing impermeable surfaces on the site.

2787097 Primary Facility 2018-10-10 Routine/Partial Inspection No Violations Noted On 10/10/18 a routine partial inspection was conducted in response to an operator request for minor modification review. The operator request for a minor modification involving adding an estimated 45' gravel reinforcement to prevent erosion on the edge of wet pond 2 was approved.
2825523 Primary Facility 2019-01-11 Compliance Evaluation No Violations Noted On 1/11/19 a compliance evaluation inspection of the Papa Bear Site was conducted in response to violations noted on 1/3/17. The following was observed:

>>No residual impact was found in the area of the spill.

>>The site was found to be in good condition with no violations found.
