There was one well on location TD’ed – locked, tagged and covered with a close-fitting waxy fabric. The rectangular cellar contained ice approximately 5’ below the lip of the cellar. Field readings for the 9-5/8” x 5-1/2” annulus were 43% methane (Altair 5X gas meter calibrated on 1/20/16). One one side, the conductor and surface casing appeared to have been removed. During drilling operations on 3/6/2010, a small hole in the intermediate casing occurred due to misalignment of the drilling derrick. A section of the casing was removed to repair this.

At 13:30 I arrived onsite and found one well onsite which was granted inactive status on 4/24/2014 (expires 4/24/2019). No other permits exist for the site. Your Well Site Restoration Information Request response letter dated 8/13/2015 indicates the site has been restored. I found the well pad remains at the full size (approx. 300’X400’) and has not been reduced in size. A spoils pile (apparently from the pit) was permanently stabilized in the northeast corner of the pad. In addition, the monument from a plugged conventional well (081-20038) was visible at the south side of the well pad. Overall, the site is currently permanently stabilized (with both vegetation and rock) but has not been restored per §3216(a) of the Oil and Gas Act. Please provide an update to your Well Site Restoration Request response letter clarifying your intentions for the site. No Violations Noted.
