1837496 | 2009-10-16 | Violation(s) Noted | Crews recently pulled off of site. The Site ID sign and E&S plan are present. The access road is rocked and silt fence was installed below the site. However, there is still mud being tracked on to the township road.
In addition, the following violation was noted: 1- This site was originally drilled in 2008. The drill cuttings were encapsulated on-site. The operator returned to the site to frac the well. Apparently while excavating for the the frac pit, crews encountered the old encapsulation from the drill pit. The integrity of the liner was compromised. At the time of the inspection, the old liner and some of the previously encapsulated drill cuttings could be seen in the excavated dirt pile. The operator was notified of the findings. The old cuttings will have to be disposed of. The most likely disposal method will be land farming. |

1864874 | 2009-12-27 | No Violations Noted | Well tag located. The well is equipped for production. The brine tank appears to be in sound condition. The site is seeded and mulched. The vegetation did begin to grow before winter set in. However, the vegetation is not fully established. The area will need to be monitored in the spring, additional seeding and mulching may be necessary. At this time, there is no evidence of accelerated erosion and sedimentation.
