Well head assembly consisted of 36¿, 24¿, 7¿, 4¿ flange connected to 2¿ production line with 5 shut off valves. No signs of leaks around well head. During inspection, DEP called Brian Haney at Interstate Gas to inquire about status of site. No pressure gauge at well head. 564582

1812537 2009-07-15 No Violations Noted At the time of inspection, no E&S or IW discharge issues at the site. During inspection, proper tag displayed at well head and gate valves were chained and locked. The 200 bbl brine tank was removed from the site and former tank location was seeded and covered with straw. Former brine tank lines have been covered with plastic and taped. According to Interstate, 4500 gallons were removed from the former brine tank. Production equipment has been removed and well has been shut off. No signs of leaks around well head.
