| Inspected well site with Josh Garland MRI and Gene Chini Oil and Gas Supervisor. Drill pit is open. Pit liner has fell into pit in multiple places. A 2 inch black plastic line has been installed between the drill pit and a 100 barrel steel production tank drain. The tank drain is equipped with a 2 inch valve, a 2 inch 90 degree elbow, a 2 inch clear plastic nipple, 2 inch steel hammer union connected to the 2 inch black plastic line that extends to the drill pit. Water was discharging from the 2 inch line into the pit. A sample was taken and sent to the lab for analysis. A High Range Quantab was placed in the discharge water and the brine exceeded 6797 parts per million chloride. Informed Tom Stone and John Kent of Mountaineer to stop discharging brine into the pit and to disconnect the 2 inch line. Also told Tom and John to pump out the pit and take the fluids to an approved salt water injection well for disposal. Then close the pit.

| Inspected well site with Josh Garland MRI and Gene Chini Oil and Gas Supervisor. Drill pit is open. Pit liner has fell into pit in multiple places. A 2 inch black plastic line has been installed between the drill pit and a 100 barrel steel production tank drain. The tank drain is equipped with a 2 inch valve, a 2 inch 90 degree elbow, a 2 inch clear plastic nipple, 2 inch steel hammer union connected to the 2 inch black plastic line that extends to the drill pit. Water was discharging from the 2 inch line into the pit. A sample was taken and sent to the lab for analysis. A High Range Quantab was placed in the discharge water and the brine exceeded 6797 parts per million chloride.  Informed Tom Stone and John Kent of Mountaineer to stop discharging brine into the pit and to disconnect the 2 inch line. Also told Tom and John to pump out the pit and take the fluids to an approved salt water injection well for disposal. Then close the pit.

The tank battery consists of 3-100 barrel steel tanks, a vertical separator, chart and meter.
1149252533 2012-01-24 Preliminary Restoration Violations Noted Preliminary restoration inspection. The pit was full of water and the liner had fallen down in places. The liner shall immediately be pulled up back into place. All pits used for drilling shall be closed two months after completion of the well. A two inch line was being run from the 210 barrel salt water tank into the pit. At the time of inspection a small amount of fluid was draining from the line and samples were taken. A high range Quan-tab test strip was used to test for Chloride levels in the water coming diectly fro the line and 6797+ parts per million Chlorides were noted. The line running from the salt water tanks to the pit shall immediately be disconnected and removed.
