| Upon inspection, it was observed that a vegetative cover has been established throughout the limits of disturbance for this pad location. Any and all drilling equipment has been removed from the location, and any and all pits used for drilling activities have been filled. This satisfies the requirements for the preliminary restoration. Accompanied on this inspection by DOGRM Engineer Jason Simmerman, and DOGRM Engineering Technicians Andy Thomas and Anthony Carson.

| Witnessed cementing of the surface casing.  Company was having trouble running casing due to parted conductor pipe.  It was parted at 86 ft. down.  The company welded straps on the collars of the pipe to ease it past the parted section of pipe.  I tested the mix water for the cement job and it showed no chlorides were present.  Good circulation and good returns were observed at the surface.

| Upon inspection, it was observed that this well is currently producing. The well is equipped with a standard production tree and is producing on the 5.5 production casing as well as the 2.875 tubing. Gauges on the well read 200 psi on the tubing and 481 psi on the casing. The tank battery consists of 6 400 bbl production tanks and 3 large mechanical separators. There is also 3 flow control separators on location as well as a large degassing tower. There is a metal, poly lined containment dike around the tank battery. Legible and correct ID information is present on the tanks as well as the entry to the lease road. No violations were observed.

| The inspection this day found that the well is producing using a plunger lift system. The valves are open on the wellhead and gas could be heard flowing through the well at the time of inspection. The tank battery for this well is equipped with six (6) 410 barrel tanks with identification posted near the tank battery. There are no issues to report and the well is in production.

0054146010 2016-01-26 Production Wells No Violations Upon inspection, it was observed that this well is currently in production. Pressure on the production tubing read 280 psi. No violations noted. I was accompanied on this inspection by DOGRM Emergency Response Coordinator Nathan Lane.
0321525386 2014-07-14 Preliminary Restoration No Violations Upon inspection, it was observed that a vegetative cover has been established throughout the limits of disturbance for this pad location.
0561409863 2014-03-27 Drill / Deepen / Reopen No Violations Checked on the status of flowback operations. There are numerous 500 barrel frac tanks on site. Spoke with contractor on site. There is no pollution or contamination on site. Well is equipped with tubing and a rabbit catcher.
0828044432 2013-11-12 Drill / Deepen / Reopen No Violations Witnessed cementing of the surface casing.
0842756662 2015-06-22 Production Wells No Violations Upon inspection, it was observed that this well is currently producing.
1318624657 2013-10-02 Drill / Deepen / Reopen No Violations Witnessed the running and cementing of the conductor pipe. 100 ft of 30 in pipe was ran and cemented to surface. Good circulation and good returns were observed at the surface.
1382620984 2013-10-23 Drill / Deepen / Reopen No Violations Drilling rig is rigging up. Spoke with company man Mr. Dave Grey. He was hoping to get started drilling by Friday 10/25/13.
1384954688 2013-11-18 Drill / Deepen / Reopen No Violations Total depth of the well during the time of inspection was 2100 ft. They were running a 12.25 in bit. I spoke with Mr. Dave Grey on site and picked up paperwork for the 13.375 in. casing cement job.
1386597356 2013-12-05 Drill / Deepen / Reopen No Violations Drilling has been completed on this well. Total depth is 12,502 ft. Company is running sweeps to stabilize the hole before they run pipe. From 8047 ft. to 12,502 ft the hole is 8.5 in. in diameter. The azimuth is 334 degrees and the inclination is 89.31 degrees as per the last downhole survey. The third party gas reading was at 1590 units.
1403360305 2017-11-27 Production Wells No Violations The inspection this day found that the well is producing using a plunger lift system. The valves are open on the wellhead and gas could be heard flowing through the well at the time of inspection. The tank battery for this well is equipped with six (6) 410 barrel tanks with identification posted near the tank battery. There are no issues to report and the well is in production.
1835399769 2013-12-09 Drill / Deepen / Reopen No Violations Picked up paperwork for the cementing of the 5.5 casing that was ran over the weekend. I received a job log, P-R graph, and a pipe tally. Spoke with company man on site.
1954889617 2013-12-02 Drill / Deepen / Reopen No Violations Total depth at the time of inspection was 8559 ft. Drillers started the curve at 7099 ft, and landed it at 7973 ft. It was reported to me as per the last survey, the inclination was 93.3 degrees and the azimuth was 334.2 degrees. Drilling was halted due to top drive issues.
2042398831 2013-11-25 Drill / Deepen / Reopen No Violations Witnessed the cementing of the 9.625 in casing. Good circulation, and good returns were witnessed at the surface. Due to cold weather, the cement contractor experienced technical difficulties. Valves and the manifold were frozen.
2063686733 2013-10-21 Drill / Deepen / Reopen No Violations Drilling rig was being assembled at the time of inspection. I called Mr. Catlan Ball from PDC energy and spoke with him about this well and others.
