| -Complaint # 619 inspection with Jeff Fry. Met with landowner Ron Wenrick. Mr. Wenrick called on 5/18/04 to report a small leak in one of the oil storage tanks near well # 20057. Small approximately 4 by 4' area of oil had leaked out over time from the northern tank. Appears that the base is rusted. Some oil in both the north and south tank according to Jeff Fry. Tanks not ID'ed nor is the well ID. Power cut 3-4 years ago to the well. Pump jack present. No meter. Valves closed off. Well incapable of production. Past inspection reports revealed that site last inspected on 2/7/04 and N.O.V.'s issued 2/14/02 for failure to ID the wells and either plug or produce the well have not been complied with. Mike Murane contacted by phone and requested to clean up the leaking tank and either plug or produce the well.

Oil leaked out to about 130-135 ft from tank. Width ranged from several feet up to 25 feet. Oil near a grassed waterway that drained into a normally dry ditch. Oil seen in old either gopher hole or field tile hole. None seen in dry creek. Landower put out straw. Mike Murnane, well owner contacted & was at site. Murnane hired ERS (Environmental Recovery Services to vacuum up the oil. Mike Weaver w/ ERS on site. Some oil, brine, water removed from S. tank (about 1/2). Oil sludge in base of N. tank. Strum Excavating hired by Murnane to build 2 trenches, one at S. end and one at N. end near tank. Murnane recommended to get roll-off container, line w/ visqueen. Murnane requested to send contaminated soil to approved landfill and grade out the area. Murnane requested to remove/repair tank. Well has no power and not capble of producing. NOV's issued on 2/14/02 for well incapable of production (1509:12); disposal of muds or cutings in violation of a rule (1509: 22C); and failure to legibly identify a well (1509: 23 & 1501:9-9-05(A)10. None of the violations have been remedied. Received complaint 619 on 5/18/04 about minor leak & well incapable of production. Nothing done by well owner other than taking out a plugging permit on 3/2/06. Permit expired on 3/2/07.  Photos taken of the leak. Initial NOV issued 2/14/02.

The well is equipped with an electric powered pump jack (electric is unhooked as previously noted) and no production has been reported to the Division since 1985. The storage consists of two silver painted 100 barrel steel tanks and a smaller steel brine tank. No legible well identification or emergency contact information is posted. Status: No change-in-status from previous inspection. Non-compliant for plug or produce and no identification written 2/7/2002. Recent attempts by the Division to locate the owner of Columbus Oilfield Exploration have been unsuccessful. Note: this is a Trempealeau openhole completion.

Site inspected on 3/22/07 and some oil that leaked from N. tank vacuumed up by ERS (Environmental Remedial Services, Inc.) About 1/2 of the oil/brine/water removed from S. tank on 3/22/07. N. tank leaked. Oil sludge in base. Strum Excavating was hired and dug 2 trenches, 1 to the S. & 1 to the N. on 3/22/07. No one on site on 3/26/07. Trackhoe has been removed. Oil contained in the 2 trenches from heavy rain on 3/23 & 3/24/07 at the site. No oil seen in dry waterway. Called Mike Murnane while on site. Mike indicated he was still trying to get a rollback container. Strum took trackhoe to another job. Requested tank be removed & area cleaned up. He said he planned to dig a trench around the tank. Notified him the well is incapable of production & should either be put into production or plug. Well & tanks are not ID'ed. Photos taken. Requested Mike to call me when he planned to have additional work done. Talked to Jeff Fry on 3/29/07 & he had been in contact w/ Mike Murnane & Ron Wenrick on 3/28/07 & work was progressing to clean up the leak. Jeff said the tank had been skidded.

Oil leaked out to about 130-135 ft from tank. Width ranged from several feet up to 25 feet. Oil near a grassed waterway that drained into a normally dry ditch. Oil seen in old either gopher hole or field tile hole. None seen in dry creek. Landower put out straw. Mike Murnane, well owner contacted & was at site. Murnane hired ERS (Environmental Recovery Services to vacuum up the oil. Mike Weaver w/ ERS on site. Some oil, brine, water removed from S. tank (about 1/2). Oil sludge in base of N. tank. Strum Excavating hired by Murnane to build 2 trenches, one at S. end and one at N. end near tank. Murnane recommended to get roll-off container, line w/ visqueen. Murnane requested to send contaminated soil to approved landfill and grade out the area. Murnane requested to remove/repair tank. Well has no power and not capble of producing. NOVs issued on 2/14/02 for well incapable of production (1509:12); disposal of muds or cutings in violation of a rule (1509: 22C); and failure to legibly identify a well (1509: 23 & 1501:9-9-05(A)10. None of the violations have been remedied. Received complaint 619 on 5/18/04 about minor leak & well incapable of production. Nothing done by well owner other than taking out a plugging permit on 3/2/06. Permit expired on 3/2/07.  Photos taken of the leak. Initial NOV issued 2/14/02.

Mr. Wenrick called on 5/18/04 to report a small leak in one of the oil storage tanks near well # 20057. Small approximately 4 by 4 area of oil had leaked out over time from the northern tank. Appears that the base is rusted. Some oil in both the north and south tank according to Jeff Fry. Tanks not IDed nor is the well ID. Power cut 3-4 years ago to the well. Pump jack present. No meter. Valves closed off. Well incapable of production. Past inspection reports revealed that site last inspected on 2/7/04 and N.O.V.s issued 2/14/02 for failure to ID the wells and either plug or produce the well have not been complied with. Mike Murane contacted by phone and requested to clean up the leaking tank and either plug or produce the well.

1175024227 2007-03-22 Production Wells Violations Noted -On site per complaint from Ron Wenrick, landowner. Mr. Wenrick called 3/22/07 & indicated one of the oil storage tanks on site had leaked. Inspection revealed N. tank leaked. Oil leaked out to about 130-135 ft from tank. Width ranged from several feet up to 25 feet. Oil near a grassed waterway that drained into a normally dry ditch. Oil seen in old either gopher hole or field tile hole. None seen in dry creek. Landower put out straw. Mike Murnane, well owner contacted & was at site. Murnane hired ERS (Environmental Recovery Services to vacuum up the oil. Mike Weaver w/ ERS on site. Some oil, brine, water removed from S. tank (about 1/2). Oil sludge in base of N. tank. Strum Excavating hired by Murnane to build 2 trenches, one at S. end and one at N. end near tank. Murnane recommended to get roll-off container, line w/ visqueen. Murnane requested to send contaminated soil to approved landfill and grade out the area. Murnane requested to remove/repair tank. Well has no power and not capble of producing. NOVs issued on 2/14/02 for well incapable of production (1509:12); disposal of muds or cutings in violation of a rule (1509: 22C); and failure to legibly identify a well (1509: 23 & 1501:9-9-05(A)10. None of the violations have been remedied. Received complaint 619 on 5/18/04 about minor leak & well incapable of production. Nothing done by well owner other than taking out a plugging permit on 3/2/06. Permit expired on 3/2/07. Photos taken of the leak. Initial NOV issued 2/14/02.
1175025989 2007-03-26 Production Wells Violations Noted -On site to follow up complaint by landowner Ron Wenrick about leaking tank first reported on 3/22/07. Site inspected on 3/22/07 and some oil that leaked from N. tank vacuumed up by ERS (Environmental Remedial Services, Inc.) About 1/2 of the oil/brine/water removed from S. tank on 3/22/07. N. tank leaked. Oil sludge in base. Strum Excavating was hired and dug 2 trenches, 1 to the S. & 1 to the N. on 3/22/07. No one on site on 3/26/07. Trackhoe has been removed. Oil contained in the 2 trenches from heavy rain on 3/23 & 3/24/07 at the site. No oil seen in dry waterway. Called Mike Murnane while on site. Mike indicated he was still trying to get a rollback container. Strum took trackhoe to another job. Requested tank be removed & area cleaned up. He said he planned to dig a trench around the tank. Notified him the well is incapable of production & should either be put into production or plug. Well & tanks are not IDed. Photos taken. Requested Mike to call me when he planned to have additional work done. Talked to Jeff Fry on 3/29/07 & he had been in contact w/ Mike Murnane & Ron Wenrick on 3/28/07 & work was progressing to clean up the leak. Jeff said the tank had been skidded.
1175702033 2007-04-03 Production Wells No Violations -On site to follow-up the work done to clean up the oil tank leak on the Wenrick property reported on 3/22/07. Since last inspection on 3/26/07, the contaminated soil has been scrapped up and removed, the trenches dug on 3/22/07 at south end & northern end of the contaminated soil has been filled back in. Dirt has been piled against the leaking tank. The tank has not been repaired or removed. Some oil seen within/under the dirt piled by the tank. Area where the oil contaminated soil was removed has been rough graded out. Photos taken of the clean up activity. Well and tanks still not IDed. Well incapable of production. Tank still has leak. NOVs were issued on 2/14/02 following a 2/7/02 inspection and the well owner (Murnane) has not brought the well into compliance.

Since the last inspection, soybeans have been planted and are growing in the area where oil had leaked out of the tank and into the field. Trenches were dug on 3/22/07, then the contaiminated soil removed. The trench was filled back in by the 4/3/07 inspection and rough graded. Dirt was piled against the tank that leaked. Tank is still present with some oil showing on the dirt piled around the tank. Well and tanks not IDed. Well incapable of production. NOVs were issued on 2/14/02 following a 2/7/02 inspection and Murnane has not brought the well into compliance.

At the time of the last inspection, soybeans were growing in the field that was affected by the late March 2007 oil leak from 1 of the 2 tanks on site. Contaminated soil was cleaned up in late March/early April 2007. Soybeans grown in 2007, in area that was contaminated have been harvested. Tank still present. Dirt piled against the tank. Some oil shown on the dirt pile but not beyond the dirt pile area. Well and tanks are not IDed. Well incapable of production. N.O.V.s were issued on 2/14/02 following a 2/7/07 inspection. Well not in compliance. Jeff Fry indicated he would contact Mike Murnane.
