| I am on site accompained by Inspector Bennett to witness a mine string cement job on HWD (Horizontal Well Drillers) Rig No. 11.  HWD drilled a 22 inch diameter hole to 191 feet and set 181 feet of 18 5/8 inch diameter, 87.5 pound casing. There are two cement baskets on this job, one at 100 feet and one at 120 feet. There is a float collar at 161 feet. Schlumberger is performing the cement job, using 185 sacks of class A 15.6 pound, 1.2 yield cement. This volume should have been enough to bring cement returns to surface, however there were no returns of any kind witnessed at surface. Next HWD placed 1 1/4 inch diameter tubing in the annular space to 60 feet, then Schlumberger pumped 61 sacks of class A cement and topped off to surface. Inspector Bennett and I witnessed good returns to surface.

HWD then ran 13 3/8 inch diameter, 54.5 pound per foot casing to a depth of 358 feet. HWD placed a shoe at 358 feet and a float collar at 319 feet. Schlumberger is preforming the cement job using 400 sacks of Class A 15.6 pound, 1.2 yield cement. Inspector Corder and I witnessed 20 bbl. of good cement returns to surface.

| On the day of my inspection accompanied by ODNR Inspector Joe Zollars, I witnessed that Horizontal Well Drillers HWR Rig #11 had drilled a 22 inch diameter hole to the depth of 191 feet drilling through a mine shaft from 136 feet to 124 feet. HWR ran 181 feet of 18.625 inch 87.5#/ft. casing with cement baskets at 120 feet and 100 feet. Schlumberger was onsite pressure tested lines, pumped 25bbls of gel, 10bbls of fresh water and 185 sacks of Class A cement with a yield of 1.2cuft/sk. Cement weight was verified with mud scales and displaced to 98 feet with 30bbls of fresh water and casing was shut-in. With no returns cement baskets were opened with two 5-gallon buckets of pea gravel and top off job was completed by Schlumberger through 60 feet of 1.25 inch tubing and pumping 61 sacks of Class A cement circulating cement to surface.

| On the day of my inspection I am viewing pad for complaint 5946. Royalty owner is concerned they are not going to get paid for production because lease is apparently transferring to Ascent resources. Pad contains 5 plunge lift wells with valves open.  Production equipment includes 5 GPUs, a compressor, and three 400 barrel steel tanks in their own containment. Surface annular pressure is 0 and intermediate annular pressure is 0. Spoke with landowner after royalty owner after inspection and he believes the situation has been corrected. Complaint can be closed.
