| On the day of my inspection I found that Phil had his water tank set and filled, and he had his clay delivered. The trailer was in place to pull tubing. Phil said that he would get the tubing out the following day.

| On the day of my inspection Phil Moran set a brush and stone bridge at depth of 575 feet. The 8.625 inch diameter casing was cut off at ground level, so Phil went in with a spear and got it set in the casing. He tried to pull the casing several times and it was not moving. Phil pulled over 40,000 pounds and locked the brake handle in to leave the casing under pull for some time. It still did not move and the weight indicator did not decrease. Phil went in with his ripping tool to start searching for a telegraph at the bottom of the casing. He came up the hole in 30-40 foot increments and finally got a telegraph at a depth of 206 feet. Phil worked on parting the casing at that depth. He got the casing parted at 206 feet and started pulling it from the well.

| On the day of my inspection I found that the well site had not been restored to its original condition. The access road to the well was fully covered in grass, but the road had been cut into the side of a crest and it had not been backfilled to meet the original contour of the surrounding landscape. The well site was graded, but it was still a little rough and there it was still bare. There were no signs that any seed had been put down to establish vegetation on the site. I found no oilfield materials or debris around the site, and there were no signs that the plugged well was leaking. The tanks had already been removed and the site restored some time before the well was plugged. I called and left a message with Dwayne Dugan to contact me in regards to the final restoration, as it was due to be completed by 4/16/18. Dwayne returned my call and we discussed the work that still needs to be completed at the well site. Dwayne confirmed that they reclaimed that tank site before the well was plugged.

I told Phil to go ahead and push some more of the remaining clay in the hole and cut the casing off.

The well site was graded, but it was still a little rough and there it was still bare. There were no signs that any seed had been put down to establish vegetation on the site. I found no oilfield materials or debris around the site, and there were no signs that the plugged well was leaking. The tanks had already been removed and the site restored some time before the well was plugged. I called and left a message with Dwayne Dugan to contact me in regards to the final restoration, as it was due to be completed by 4/16/18. Dwayne returned my call and we discussed the work that still needs to be completed at the well site. Dwayne confirmed that they reclaimed that tank site before the well was plugged.
1835066944 2017-09-20 Plug / Plug Back No Violations On the day of my inspection I found that Phil Moran was rigged up to plug the well. He already had the rods out and still needed to pull the tubing. The well will be plugged with fire clay. I spoke with Phil to ensure that he was aware of the recent change to the approved clay sources in Ohio. There was an empty 100 barrel water tank on site.
2011164490 2017-10-13 Plug / Plug Back No Violations On the day of my inspection I found that the service rig and all of the equipment were still on location. The small clay pile was still in place and the hole was covered with a plate. I looked into the well and found the fluid had receded, leaving behind the actual clay top. I estimated that the clay was down 10 to 15 feet. I called Phil Moran and he said that he has been unable to work due to health issues. He said that he wanted to get his equipment out before Sunday to avoid the rain. I told Phil to go ahead and push some more of the remaining clay in the hole and cut the casing off.
