0062879869 | 2017-09-21 | Plug / Plug Back | No Violations | On the day of my inspection I found that Phil had his water tank set and filled, and he had his clay delivered. The trailer was in place to pull tubing. Phil said that he would get the tubing out the following day. |
0260256119 | 2017-10-04 | Plug / Plug Back | No Violations | On the day of my inspection Phil Moran set a brush and stone bridge at depth of 575 feet. The 8.625 inch diameter casing was cut off at ground level, so Phil went in with a spear and got it set in the casing. He tried to pull the casing several times and it was not moving. Phil pulled over 40,000 pounds and locked the brake handle in to leave the casing under pull for some time. It still did not move and the weight indicator did not decrease. Phil went in with his ripping tool to start searching for a telegraph at the bottom of the casing. He came up the hole in 30-40 foot increments and finally got a telegraph at a depth of 206 feet. Phil worked on parting the casing at that depth. He got the casing parted at 206 feet and started pulling it from the well. |
0456928778 | 2017-09-26 | Plug / Plug Back | No Violations | On the day of my inspection I found that Phil Moran was trying to rip the 4.5 inch diameter casing at a depth of 3503 feet. Phil claimed that they washed 2 tons of clay down the casing on Saturday. Phil worked at ripping the casing for a while with no luck. He came up the hole 100 feet and got a better telegraph. |
0602711158 | 2018-07-13 | Final Restoration | No Violations | On the day of my inspection I found that the access road berms had been graded down to a point that allows for the hay to be mowed. The entire area was 100 percent covered in hayfield grasses. There were no signs of erosion, and there were no signs that the plugged well was leaking. This site passes final restoration. |
0740098836 | 2017-10-16 | Plug / Plug Back | No Violations | On the day of my inspection I found that all equipment and debris had been removed except one bulldozer. The remaining clay pile was gone, the well was covered up and the site was rough graded. I saw the steel plate that Phil was going to use when I was at the site on 10/13/17. This well is plugged and abandoned. |
0899484262 | 2015-01-27 | Production Wells | No Violations | On the day of my inspection I found that there was no identification posted for this well. I called Travis Dugan to give him a verbal warning that identification must be posted within 30 days. Failure to do so will result in the issuance of a compliance notice. I emailed him a list of wells that need to be identified. |
1105294586 | 2017-09-28 | Plug / Plug Back | No Violations | On the day of my inspection Phil Moran finished pulling the 4.5 inch diameter casing from the well. There was 7 inch casing visible at the surface. Phil speculated that the records were wrong and the 7 inch casing was left in the well rather than the 8.625 inch. Phil set a brush and stone bridge at 1280 feet in the case that he had problems with pulling the 7 inch casing. |
1275351012 | 2017-10-02 | Plug / Plug Back | No Violations | On the day of my inspection I found that Phil Moran was trying to rip the 7 inch diameter casing. Phil said that he also found that there was 8.625 inch casing in the well when he dug down to weld a 7 inch nipple on. Phil had a telegraph at 1190 feet and he started trying to part the casing at that depth. Phil got the 7 inch casing ripped at 1190 feet. He started pulling the casing from the well. |
1426598147 | 2015-03-17 | Production Wells | No Violations | On the day of my inspection I found that proper identification was posted. This well was in compliance with OAC 9-9-05(A)10. |
1506351964 | 2017-09-22 | Plug / Plug Back | No Violations | On the day of my inspection I found that Phil Moran was still pulling the tubing from the well. He planned to get the bottom plug started as soon as the tubing was out. I was called away to another job, so I made sure that Phil was aware of the amount of clay that I wanted on the bottom plug. He also said that he might be out to work on Saturday. |
1506604768 | 2017-09-27 | Plug / Plug Back | No Violations | On the day of my inspection I found that Phil Moran had the 4.5 inch casing ripped of at 3400 feet. Phil pulled 20 joints and then washed 6 tons of clay in the well for a total of 8 tons on the bottom plug. |
1507125006 | 2017-10-03 | Plug / Plug Back | No Violations | On the day of my inspection Phil Moran pulled the 7 inch diameter casing from the well. Then he washed 4 tons of clay on top of the brush and stone bridge to get a 200 foot plug across the Berea sandstone. Phil started to work on the 8.625 inch casing, but decided to shut down and haul the 7 inch to be tested for future use. Phil was concerned that he would not have enough clay to plug the well to surface from the brush and stone set at 1275 feet. He was also concerned that he would not be able to get a pre-cast tapered plug down to set on the 8 inch seat. I told Phil that he could set another brush and stone bridge below the surface casing before he pulled it. |
1507229752 | 2017-10-05 | Plug / Plug Back | No Violations | On the day of my inspection I found that Phil Moran had pulled the free 8.625 inch casing from the well. Phil was ready to rig down and move his rig off of the well before washing the remainder of the clay in for the surface plug. It started raining fairly heavily, so Phil decided to hold off on doing any more work until tomorrow. |
1507312993 | 2017-10-06 | Plug / Plug Back | No Violations | On the day of my inspection I found that Phil had taken the truck and trailer with the 8.625 inch casing away from the site. I called Phil and he said that he was going to wait another day to finish up because of the rain. I asked Phil to leave the well open if he fills it over the weekend or on the holiday, and I would be out first thing Tuesday morning. |
1507645384 | 2017-10-10 | Plug / Plug Back | No Violations | On the day of my inspection I found that most of the remaining 10 tons of clay had been washed down the well. There was approximately one half ton left piled around the top of the well bore. There was a steel plate covering the hole. I removed the plate to look into the well. I could see the fluid top approximately 10 to 15 feet down. Phi Moran was not at the site, so I called him. He said that he was probably not going to be out today to finish topping the clay off. I asked Phil to call me when he plans to come back and finish up. |
1646257832 | 2018-05-08 | Final Restoration | No Violations | On the day of my inspection I found that the well site had not been restored to its original condition. The access road to the well was fully covered in grass, but the road had been cut into the side of a crest and it had not been backfilled to meet the original contour of the surrounding landscape. The well site was graded, but it was still a little rough and there it was still bare. There were no signs that any seed had been put down to establish vegetation on the site. I found no oilfield materials or debris around the site, and there were no signs that the plugged well was leaking. The tanks had already been removed and the site restored some time before the well was plugged. I called and left a message with Dwayne Dugan to contact me in regards to the final restoration, as it was due to be completed by 4/16/18. Dwayne returned my call and we discussed the work that still needs to be completed at the well site. Dwayne confirmed that they reclaimed that tank site before the well was plugged. |
1835066944 | 2017-09-20 | Plug / Plug Back | No Violations | On the day of my inspection I found that Phil Moran was rigged up to plug the well. He already had the rods out and still needed to pull the tubing. The well will be plugged with fire clay. I spoke with Phil to ensure that he was aware of the recent change to the approved clay sources in Ohio. There was an empty 100 barrel water tank on site. |
2011164490 | 2017-10-13 | Plug / Plug Back | No Violations | On the day of my inspection I found that the service rig and all of the equipment were still on location. The small clay pile was still in place and the hole was covered with a plate. I looked into the well and found the fluid had receded, leaving behind the actual clay top. I estimated that the clay was down 10 to 15 feet. I called Phil Moran and he said that he has been unable to work due to health issues. He said that he wanted to get his equipment out before Sunday to avoid the rain. I told Phil to go ahead and push some more of the remaining clay in the hole and cut the casing off. |
