+ | | I was on the the site of the Summa # 1-D ( P&A in 2008 ) to meet with Robert Kroupa ( Director of Facilites Construction ) and Mike Wohlwend ( Wohlwend Engineering ) to review Summas construction plans for the site on which the well was located. My inspection revealed that the wellhead was sticking up above grade. There were not any indications of any leakage from the wellhead. Mr. Kroupa previously had asked what requirements that DOGRM had regarding construction over a plugged and abandoned well. I replied that while DOGRM did not have any rules or requirements addressing such it is advised that the placement of a structure over a plugged well should be avoided, if possible. We discussed the placement of a vaulted and venting system over the well. I suggested that Mr. Kroupa contact a Engineering firm familiar with construction and operation of such a system. Mr. Kroupa said, that it is possilble that the new construction could be oriented to miss the plugged well by a few feet. It was agreed by all that this avenue would be the best remedy. I suggested that even if the plugged is outside the confines of the structure that it be vaulted and vented above the roof level. I requested that they contact me as future plans develop. | ||

+ | | I was on the Summa # 1- D location to observe the cutting off of the 8.625 surface casing and installation of the permanent concrete vault. Moore Well Services was the contactor for this procedure engaged by Donley / Shook ( General Contractors for Summa Medical System ). The well was originally plugged in March of 2008. The plugging involved four (4) cement plugs across various depth intervals in the borehole and a wireline bridge plug in the 8.625 surface casing. The 8.625 surface casing was cut off approximately two (2) feet above the excavated grade. Brandon Williams ( Rettew Engineering ) was testing and documenting the casing stump for any gas migration. After the surface cement sheath was removed, from the casing, Mr. Williams also monitored the remnant cement for any migration. No gas was detected. A gravel pad approximately 12 thick was placed down and graded. A prefab concrete catch basin was lowered over the casing and leveled. A concrete lid was placed on top of the basin and sealed. The vault will have two (2) vent line opened to the outside of the structure. The first was connected, via a dresser sleeve , to the 8.625 surface casing directly and sealed. The second will monitor the ambient air in the vault for any leakage. An identifying cap ( ODNR and permit number ) was attached to the 8.625 casing. This data was also welded onto the steel manhole lid which was to be visible after the cement pour of the basement floor. This plugged and abandoned well was in compliance with the Final Restoration criteria and passed. No violations identified during this inspection. | ||

0743953962 | 2016-05-19 | Plug / Plug Back | No Violations | I was on the the site of the Summa # 1-D ( P&A in 2008 ) to meet with Robert Kroupa ( Director of Facilites Construction ) and Mike Wohlwend ( Wohlwend Engineering ) to review Summas construction plans for the site on which the well was located. My inspection revealed that the wellhead was sticking up above grade. There were not any indications of any leakage from the wellhead. Mr. Kroupa previously had asked what requirements that DOGRM had regarding construction over a plugged and abandoned well. I replied that while DOGRM did not have any rules or requirements addressing such it is advised that the placement of a structure over a plugged well should be avoided, if possible. We discussed the placement of a vaulted and venting system over the well. I suggested that Mr. Kroupa contact a Engineering firm familiar with construction and operation of such a system. Mr. Kroupa said, that it is possilble that the new construction could be oriented to miss the plugged well by a few feet. It was agreed by all that this avenue would be the best remedy. I suggested that even if the plugged is outside the confines of the structure that it be vaulted and vented above the roof level. I requested that they contact me as future plans develop. |

2114254430 | 2017-08-24 | Final Restoration | No Violations | I was on the Summa # 1- D location to observe the cutting off of the 8.625 surface casing and installation of the permanent concrete vault. Moore Well Services was the contactor for this procedure engaged by Donley / Shook ( General Contractors for Summa Medical System ). The well was originally plugged in March of 2008. The plugging involved four (4) cement plugs across various depth intervals in the borehole and a wireline bridge plug in the 8.625 surface casing. The 8.625 surface casing was cut off approximately two (2) feet above the excavated grade. Brandon Williams ( Rettew Engineering ) was testing and documenting the casing stump for any gas migration. After the surface cement sheath was removed, from the casing, Mr. Williams also monitored the remnant cement for any migration. No gas was detected. A gravel pad approximately 12 thick was placed down and graded. A prefab concrete catch basin was lowered over the casing and leveled. A concrete lid was placed on top of the basin and sealed. The vault will have two (2) vent line opened to the outside of the structure. The first was connected, via a dresser sleeve , to the 8.625 surface casing directly and sealed. The second will monitor the ambient air in the vault for any leakage. An identifying cap ( ODNR and permit number ) was attached to the 8.625 casing. This data was also welded onto the steel manhole lid which was to be visible after the cement pour of the basement floor. This plugged and abandoned well was in compliance with the Final Restoration criteria and passed. No violations identified during this inspection. |
