At the time of this inspection, the fencing around the well has not been installed, and it was not waived during the pre-site inspection. The pick-up lines have plugs, and the handles have been removed. The 210 BBL storage tank was missing the required urban safety equipment, except for a charcoal filter. It was observed that small holes had started to form near the top of the 210 BBL tank and some staining could be seen down the side of the tank. No active leak was observed from the 210 BBL tank, at the time of this inspection. There was an identification sign posted at the tank battery, that still needed the required information per 1501:9-9-05(E)(1)(a-e). There was a wooden fence around the tank battery but the door was missing from the fence, and multiple boards have fallen, leaving large areas of the tank battery unsecure. At the time of this inspection, the fencing around the well has not been installed, and it was not waived during the pre-site inspection. The pick-up lines have plugs, and the handles have been removed. The 210 BBL storage tank was missing the required urban safety equipment, except for a charcoal filter. It was observed that small holes had started to form near the top of the 210 BBL tank and some staining could be seen down the side of the tank. No active leak was observed from the 210 BBL tank, at the time of this inspection. There was an identification sign posted at the tank battery, that still needed the required information per 1501:9-9-05(E)(1)(a-e). There was a wooden fence around the tank battery but the door was missing from the fence, and multiple boards have fallen, leaving large areas of the tank battery unsecure. The 210 BBL storage tank extends more than 3 foot above the wooden fencing. Landscaping has not been implemented at the well or tank battery.
PAC needs to: 1) Correct the identification sign to be in compliance with 1501:9-9-05(E)(1)(a-e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 2) Install urban safety equipment on the production tank as required by 1501:9-9-05(E)(4)(b-e): Functioning low-pressure relief valve. Tank hatch lids shall have a functioning seal and the hatch shall be secured at all times. Functioning lightning arrestor. 3) Replace the 210 BBL tank with holes at the top. 4) Repair the existing fence around the tank battery and reinstall the man door with a lock. 5) Install fencing around the well. |

The tank battery has two storage tanks and a separator. The pick-up lines have plugs, but are missing locks. The 210 BBL storage tank was missing the required urban safety equipment.
