| On 1/30/2013, I witnessed Universal stimulate the Lipka Well for PAC Drilling.  PAC treated with 500 gallons of acid for one hour.  The bore hole was loaded up with 3,196 gallons of fresh water.  Universal start pumping at 20 bbl/min and increased the rate 10 bbl/min topping at 50 bbl/min.  A total amount of freshwater pumped was 135,450 gallons and 25 balls were dropped.

| I conducted an inspection on this well site location.  At the time of this inspection, the well was equipped with a plunger lift assembly and in production.  The gate valve on the tubing and the valve on the 2 inch flow line was open.  There is identification posted at the well.  The diked tank battery has one 210 BBL production tank, one 50 BBL poly brine tank and a mechanical separator.  The valves on the separator were open and the chart on the separator showed production.  The tank valve and pick-up lines have plugs and their handles have been removed.  The required urban safety equipment has been installed on the production tank.  The hatch on top of the production tank is not secured.  An identification sign was posted at the tank battery, but needs amended with the required information per 1501:9-9-05(E)(1)(a-e).  There is no landscaping at the tank battery and well; however, they are both located over 680 feet from Warner Road and hidden by trees and vegetation.  Fencing around the well and tank battery was waived on the pre-site inspection.  The lease access stable, however the access gate has not been installed.  The gate was not waived during the pre-site inspection.

| My inspection found the site graded and seeded.  Vegetation was well established.  The tank battery contained a 210 barrel production tank, a 50 barrel brine tank, and a separator.  There was a 7 day production chart in the separator.  The well was equipped with a plunger lift unit.  All debris has been removed.  The pits were filled and closed.  The proper identification sign was posted.  The proper safety equipment was installed on the production tank.

2013-01-30 ACID 0 500 Gal 0 Universal Well Services Inc. On 1/30/2013, I witnessed Universal stimulate the Lipka Well for PAC Drilling. PAC treated with 500 gallons of acid for one hour. The bore hole was loaded up with 3,196 gallons of fresh water. Universal start pumping at 20 bbl/min and increased the rate 10 bbl/min topping at 50 bbl/min. A total amount of freshwater pumped was 135,450 gallons and 25 balls were dropped.

An identification sign is posted at the tank battery. The identification sign is missing the required information per 1501:9-9-05(E)(1)(a--site inspection due to the hidden location. The access road is in good repair with no gate at the entrance. The gate was not waived during the pre-site inspection.

PAC needs to: 1) Correct the identification sign to be in compliance with 1501:9-9-05(E)(1)(a-e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 2) Install a gate with the proper identification sign at the access road entrance per 1501:9-9-05(E)(1)(a-f) & 1501:9-9-05(E)(2)(c). 3) Secure the hatch on the production take as required by 1501:9-9-05(E)(4)(d). 4) Install locks on the pick-up lines or remove the valve handles as required by 1501:9-9-05(E)(3).

0726280296 2019-03-01 UrbanProduction Wells Violations Noted I conducted an inspection on this well site location. At the time of this inspection, the well was equipped with a plunger lift assembly and in production. The gate valve on the tubing and the valve on the 2 inch flow line was open. There is identification posted at the well. The diked tank battery has one 210 BBL production tank, one 50 BBL poly brine tank and a mechanical separator. The valves on the separator were open and the chart on the separator showed production. The tank valve and pick-up lines have plugs and their handles have been removed. The required urban safety equipment has been installed on the production tank. The hatch on top of the production tank is not secured. An identification sign was posted at the tank battery, but needs amended with the required information per 1501:9-9-05(E)(1)(a-e). There is no landscaping at the tank battery and well; however, they are both located over 680 feet from Warner Road and hidden by trees and vegetation. Fencing around the well and tank battery was waived on the pre-site inspection. The lease access stable, however the access gate has not been installed. The gate was not waived during the pre-site inspection.

They displaced with 8 barrels of fresh water to a depth of 64. There was 5 barrels of good cement return to surface. The cement held at surface for 20 minutes. The glacial till depth was estimated at 18.
