| I was on location with inspector John Fleming.  We were checked by security before we entered the site.  The lease road was stable and drainage controls were in place.  Operator had recently repaired the drainage along the west side of the lease road, north end with mesh and seeded the area. The well is currently shut in and being prepped for flow testing.  Operator was working on the construction of the tank battery and the separator units.  The two impoundments on location were fenced.  Operator is planning on reclaiming the north impoundment at the time of inspection it was being pumped down.  No violations on location.

| On 6/10/2012, I arrived on location at 3:45 pm. They were trying to install the float shoe on the bottom of the 20 casing, on the rig floor but kept getting cross threaded. They lowered the casing down to the pipe floor and installed it there. They started running the 20 casing and could only get 4 joint 160 in the 26 bore hole. They hooked Cal-Frac up to the casing pipe and tried to circulate down but only dropped 7 They decided to pull it back out and go in with a Tri-Cone bit and rimer and drill to 388.

There were vacuumed trucks on location at that time they responded with-in minutes. On 1/24/2013, I met with Ken Cerny and Andrew Sarver who were on location when this occrued. There was a plug on the manifold line that broke . The Primary containment size is 20 deep by 50' long and 40' wide. This was also setting on a larger containment pad. No release was on soil. All flow back is properly disposed of. I inspected the area and there is no violation.

| The purpose of my inspection was to document the status of workover operations on well #2-3172-01.  At the time of my inspection the operator had shut the well in and disconnected the flow line from the well head.  Correct and legible identification was present.  Containment was placed around the well head.  A weir tank and 500 bbl frac tank were also on location and spotted on containment.  No workover rig or personnel were present.  No evidence of spills or leaks were observed.  No violations were observed during this inspection.

| The purpose of my inspection was to witness workover/ well service operations on well #2-3172-01-00.  A Basic Energy Snubbing Unit was on location pulling tubing from the well to install a cross over nipple to convert the well from open flow to a plunger lift assembly.  Newfield consultant Greg Friar and Consol Energy Representative Ron Kinsey were on location supervising operations.  Force Incorporated delivered 9.4 pound per gallon brine from Consol production wells in Noble County per Mr. Friar.  An operating blow out preventer stack containing pipe rams and a blind shear was nippled up to the well head.  I did not observe any spills or leaks off of containment.  No violations were observed during this inspection.

The lease road was stable and drainage controls were in place. Operator had recently repaired the drainage along the west side of the lease road, north end with mesh and seeded the area. The well is currently shut in and being prepped for flow testing. Operator was working on the construction of the tank battery and the separator units. The two impoundments on location were fenced. Operator is planning on reclaiming the north impoundment at the time of inspection it was being pumped down. No violations on location.
0264015838 2012-06-10 Drill / Deepen / Reopen No Violations On 6/10/2012, I arrived on location at 3:45 pm. They were trying to install the float shoe on the bottom of the 20 casing, on the rig floor but kept getting cross threaded. They lowered the casing down to the pipe floor and installed it there. They started running the 20 casing and could only get 4 joint 160 in the 26 bore hole. They hooked Cal-Frac up to the casing pipe and tried to circulate down but only dropped 7 They decided to pull it back out and go in with a Tri-Cone bit and rimer and drill to 388.

The rig crew was trying to install the float shoe on the bottom of the 20 casing, after several attempts they successfully installed the shoe and started running the 20 casing. They were able to get 4 joints ( 160 ) in the well before it stopped. They hooked up Cal Frac to circulated down the 20 casing but only dropped 7. They decided to pull casing back out and go in with a Tri-Cone bit and open up the well bore to 388.

When everybody was in their operating areas they began to start the frac operation. The average pressure was 7796, the average barrels pumped per minute were 77 with a total of 7122 barrels were used. Job was completed.
1359647699 2013-01-24 Completion Testing No Violations On 1/23/2013, I received a phone call from Ken Cerny, Environmental Enforcement for C N X Energy informing me that they had a 120 barrel releases of flow back water into a contained area. There were vacuumed trucks on location at that time they responded with-in minutes. On 1/24/2013, I met with Ken Cerny and Andrew Sarver who were on location when this occrued. There was a plug on the manifold line that broke . The Primary containment size is 20 deep by 50 long and 40 wide. This was also setting on a larger containment pad. No release was on soil. All flow back is properly disposed of. I inspected the area and there is no violation.

Inspector Ken Fry will cover the job I will be on vacation that week. At this time there is no violations.
1497460824 2017-06-12 Work Over Reconditioning No Violations The purpose of my inspection was to document the status of workover operations on well #2-3172-01. At the time of my inspection the operator had shut the well in and disconnected the flow line from the well head. Correct and legible identification was present. Containment was placed around the well head. A weir tank and 500 bbl frac tank were also on location and spotted on containment. No workover rig or personnel were present. No evidence of spills or leaks were observed. No violations were observed during this inspection.
1497633299 2017-06-16 Work Over Reconditioning No Violations The purpose of my inspection was to witness workover/ well service operations on well #2-3172-01-00. A Basic Energy Snubbing Unit was on location pulling tubing from the well to install a cross over nipple to convert the well from open flow to a plunger lift assembly. Newfield consultant Greg Friar and Consol Energy Representative Ron Kinsey were on location supervising operations. Force Incorporated delivered 9.4 pound per gallon brine from Consol production wells in Noble County per Mr. Friar. An operating blow out preventer stack containing pipe rams and a blind shear was nippled up to the well head. I did not observe any spills or leaks off of containment. No violations were observed during this inspection.
