| I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Precision Rathole has drilled a 26 inch hole to a depth of 60 feet and then drilled a 24 inch hole to a total depth of 113 feet. They ran 113 feet of 20 inch conductor casing (94 pound per foot with an inside diameter of 19.124 inches) into the well bore. O-TEX cemented the casing with 210 sacks of ClassA cement with 2% CaCl, 1/4 pound per sack cello-flake, and 0.4% C-41P. The cement slurry had a density of 15.6 pounds per gallon, a yield of 1.20 cubic feet per sack, and they used 5.25 gallons of mix water per sack. O-TEX pumped 44.88 barrels of cement slurry, pumped 36.5 barrels of displacement, and there was 10 barrels of good cement returns. My inspection revealed no violations at this time

| I was on site to witness the cementing of the 13 3/8 inch surface casing. My inspection revealed that Nomac Rig # 77 had drilled a 17.5 inch hole to a total depth of 476 feet and ran 476 feet of 13 3/8 inch surface casing (54.5 pounds per foot / inside diameter 12.615 inches). They installed 4 bowspring centralizers on the casing string on joints 1,2,5, and 8. Schlumberger cemented the casing with 440 sacks of Class A (D901) with 2% D154, 0.2% D046, 0.15% B160A, 0.5% B348, 0.6% S001, and .125 pound per sack D130. The cement had a density of 15.6 pounds per gallon, a yield of 1.21 cubic feet per sack, and they used 5.238 gallons of mix water per sack. Schlumberger pumped 94.8 barrels of cement, 67.5 barrels of displacement, and there was 22 barrels of cement returns to surface. They bumped the plug at 12:37 pm with 311 PSI and the bumped the pressure up to 1368 PSI to conduct the casing test. Casing test was approved, floats held, and there was approximately 1/2 barrel of flowback when the pressure was released. My inspection revealed no violations at this time.

| I was on location to conduct a preliminary restoration inspection.  The well is equipped with a plunger lift unit.  Jersey barriers were installed around the wellhead.  The well is properly identified with a clear, legibly-marked well identification sign.  The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment.  There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators.  There is a truck grounding area for trucks collecting condensate and/or produced water.  Lightning protection and a windsock are installed on top of the production tanks.  Other production equipment on the well pad consisted of:

Jersey barriers are installed around the production equipment.  The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked.  Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation.  Drainage and erosion controls at the pad are in place and functioning.  A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure.  The pad has a crushed limestone surface, and is in good repair.  The lease road is stable and drivable.  Preliminary restoration passed.

| I conducted a status check of this well.  Identification signs were posted at the access road entrance, the well, and the storage tanks.  The access road was equipped with a gate and was locked.  The #8H well is in production. The well is equipped with a plunger lift assembly.  The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

| I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Precision Rathole has drilled a 26 inch hole to a depth of 60 feet and then drilled a 24 inch hole to a total depth of 113 feet. They ran 113 feet of 20 inch conductor casing (94 pound per foot with an inside diameter of 19.124 inches) into the well bore. O-TEX cemented the casing with 210 sacks of ClassA cement with 2% CaCl, 1/4 pound per sack cello-flake, and 0.4% C-41P. The cement slurry had a density of 15.6 pounds per gallon, a yield of 1.20 cubic feet per sack, and they used 5.25 gallons of mix water per sack. O-TEX pumped 44.88 barrels of cement slurry, pumped 36.5 barrels of displacement, and there was 10 barrels of good cement returns. My inspection revealed no violations at this time

| I was on site to witness the cementing of the 13 3/8 inch surface casing. My inspection revealed that Nomac Rig # 77 had drilled a 17.5 inch hole to a total depth of 476 feet and ran 476 feet of 13 3/8 inch surface casing (54.5 pounds per foot / inside diameter 12.615 inches). They installed 4 bowspring centralizers on the casing string on joints 1,2,5, and 8. Schlumberger cemented the casing with 440 sacks of Class A (D901) with 2% D154, 0.2% D046, 0.15% B160A, 0.5% B348, 0.6% S001, and .125 pound per sack D130. The cement had a density of 15.6 pounds per gallon, a yield of 1.21 cubic feet per sack, and they used 5.238 gallons of mix water per sack. Schlumberger pumped 94.8 barrels of cement, 67.5 barrels of displacement, and there was 22 barrels of cement returns to surface. They bumped the plug at 12:37 pm with 311 PSI and the bumped the pressure up to 1368 PSI to conduct the casing test. Casing test was approved, floats held, and there was approximately 1/2 barrel of flowback when the pressure was released. My inspection revealed no violations at this time.

0514991707 2015-10-23 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0814812045 2017-03-07 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road was equipped with a gate and was locked. The #8H well is in production. The well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consists of the following: 4 producing wells

1 Low pressure vertical separator located inside of dike 4 sand separators 4 gas processing units 1 vapor recovery unit 1 heat treater 1 compressor station There was approximately four inches of standing water inside of the dike. I tested the standing water with a low-range QuanTab which concluded in a reading of under 30 ppm.

1294488332 2013-09-22 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Precision Rathole has drilled a 26 inch hole to a depth of 60 feet and then drilled a 24 inch hole to a total depth of 113 feet. They ran 113 feet of 20 inch conductor casing (94 pound per foot with an inside diameter of 19.124 inches) into the well bore. O-TEX cemented the casing with 210 sacks of ClassA cement with 2% CaCl, 1/4 pound per sack cello-flake, and 0.4% C-41P. The cement slurry had a density of 15.6 pounds per gallon, a yield of 1.20 cubic feet per sack, and they used 5.25 gallons of mix water per sack. O-TEX pumped 44.88 barrels of cement slurry, pumped 36.5 barrels of displacement, and there was 10 barrels of good cement returns. My inspection revealed no violations at this time
1390322332 2014-01-21 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 inch surface casing. My inspection revealed that Nomac Rig # 77 had drilled a 17.5 inch hole to a total depth of 476 feet and ran 476 feet of 13 3/8 inch surface casing (54.5 pounds per foot / inside diameter 12.615 inches). They installed 4 bowspring centralizers on the casing string on joints 1,2,5, and 8. Schlumberger cemented the casing with 440 sacks of Class A (D901) with 2% D154, 0.2% D046, 0.15% B160A, 0.5% B348, 0.6% S001, and .125 pound per sack D130. The cement had a density of 15.6 pounds per gallon, a yield of 1.21 cubic feet per sack, and they used 5.238 gallons of mix water per sack. Schlumberger pumped 94.8 barrels of cement, 67.5 barrels of displacement, and there was 22 barrels of cement returns to surface. They bumped the plug at 12:37 pm with 311 PSI and the bumped the pressure up to 1368 PSI to conduct the casing test. Casing test was approved, floats held, and there was approximately 1/2 barrel of flowback when the pressure was released. My inspection revealed no violations at this time.
