| SPILL    Operator self reported a spill on 1/22/2014, at the Chevron Porter-Naylor 27H well in Perry Twp., Fayette County.  The spill was reported by Beth Smock at 14:45, the incident occurred at 13:42 the same day.

According to documumentation and reports from Chevron- a blender ran out of fuel and as a result of the blender shutdown, an overflow of1 barrel of frac fluid took place into secondary containment.  While refueling the blender, the PCM unit was overfilled allowing another 1 barrel of frac fluid to be released into secondary containment.  Cleanup activities commenced immediately.  Materials recovered with a vacum unit were returned to the system and reused.  Documentation provided to the Department is attached to this report.

| On 05/15/15 at approximately 1000 hours I along with WQS Sam Steingrabe met with representatives of Chevron Appalachia LLC to discuss CFS removal at the Porter-Naylor Unit.The site had recently been graded and hydroseeded. Small sprouts were visible on the site. At the bottom right hand side of the access road nearest to the pad was a CFS that Chevron wanted approval to remove. There was a sufficient vegetative strip surrounding the CFS and I approved the removal and redlined the E&S plans in the field. Along the left hand side of the access road were CFSs that were improperly maintained due to dirt being pushed up against them. It was recommended that Chevron address these issues. There were no violations noted at the time of inspection

2244963 2014-02-13 In Compliance with Policy SPILL Operator self reported a spill on 1/22/2014, at the Chevron Porter-Naylor 27H well in Perry Twp., Fayette County. The spill was reported by Beth Smock at 14:45, the incident occurred at 13:42 the same day.

According to documumentation and reports from Chevron- a blender ran out of fuel and as a result of the blender shutdown, an overflow of1 barrel of frac fluid took place into secondary containment. While refueling the blender, the PCM unit was overfilled allowing another 1 barrel of frac fluid to be released into secondary containment. Cleanup activities commenced immediately. Materials recovered with a vacum unit were returned to the system and reused. Documentation provided to the Department is attached to this report.

Uponn review of all the information in relation to this spill incident, the operator performed the proper spill reporting requirements and has reported sufficient remediation at the area of release in compliance with the Departments current Spill Policy.

2373167 2015-05-15 No Violations Noted On 05/15/15 at approximately 1000 hours I along with WQS Sam Steingrabe met with representatives of Chevron Appalachia LLC to discuss CFS removal at the Porter-Naylor Unit.The site had recently been graded and hydroseeded. Small sprouts were visible on the site. At the bottom right hand side of the access road nearest to the pad was a CFS that Chevron wanted approval to remove. There was a sufficient vegetative strip surrounding the CFS and I approved the removal and redlined the E&S plans in the field. Along the left hand side of the access road were CFSs that were improperly maintained due to dirt being pushed up against them. It was recommended that Chevron address these issues. There were no violations noted at the time of inspection
