On 06/17/16 at approximately 1030 hours I conducted a CEI at the Porter Naylor site. The Act 9 sign was in place and all required documents were present at the site. On 02/04/14 the Department conducted an inspection of the site and found violations of chapter 105 for failing to have the GP-7 permit present at the crossing and for failing to construct the stream crossing as approved. A violation of 102.8 was cited for failure to obtain PCSM for a paved apron at the access road entrance. At the time of inspection the paved apron had been removed and the stream crossing had been reworked to be compliant with the approved GP-7. Chevron has corrected the violations cited in the 02/04/14 inspection report. There were no violations observed at the time of inspection. |
