| At the time of inspection the well appears to be dilled and equipped for production.  The well apparatus and brine tanks located onsite appear to in sound working condition.  The Department notes accelerated erosion created from run off of the adjacent hillside is creating a scoured ditch to transport erosion and sedimentation to be transported over the hillside behind the well head.  The Department suggest that the Operator line the ditch at the bottom of the hill adjacent to the well head with clean durable rock.  The Department also suggest the Operator regrade the affected area and install rip rap and catch basin between the well pad area down to the sediment fencing installed at the limit of disturbance.

Cement returns to surface not realized with 30' of cement left in hole at 20:11 01-31-10. Grouted backside with 9 bbls of cement. Cement basket set at 378' as proposal indicated potential mine at 406'-436'. Open mine not encountered. Drilling pit lined and freeboard maintained. E/S controls in place and functioning as per available plan.

Cement returns to surface realized with plug landing at 22:07 02-02-11. Drilling pit lined and freeboard maintained. E/S controls in place and functioning as per available plan.

1949787 2011-02-02 No Violations Noted Rig is on-site. Site ID sign and E&S plan located. Pit appears to be in sound condition. There is no evidence of accelerated erosion and sedimentation on the well site or the access road.

2044772 2012-02-23 No Violations Noted Well is located on the same well pad as 005-30550. Brine tanks and seperator apparatus located onsite and appeared to be in sound condition. There was no observed accelerated erosion or sedimenation in relation to the access road or well-site. Site and freshwater impoundmnet had been regraded, seeded and mulched. The oprator should continue to monitor the site to insure that 70% uniform coverage criteria.
2069983 2012-05-22 No Violations Noted At the time of inspection the well appears to be dilled and equipped for production. The well apparatus and brine tanks located onsite appear to in sound working condition. The Department notes accelerated erosion created from run off of the adjacent hillside is creating a scoured ditch to transport erosion and sedimentation to be transported over the hillside behind the well head. The Department suggest that the Operator line the ditch at the bottom of the hill adjacent to the well head with clean durable rock. The Department also suggest the Operator regrade the affected area and install rip rap and catch basin between the well pad area down to the sediment fencing installed at the limit of disturbance.
2201402 2013-09-04 No Violations Noted Well appeared to be equipped for production. Brine tanks located on-site and they appeared to be in sound condition. Site had been properly restored and the vegetation met the 70% uniform coverage criteria. There was no evidence observed of accelerated erosion or sedimentation in relation to the site or access road.
