+ | | Upon inspection, Stonebridge Operating Co's roust crew was on location and working on cleaning up the oil spill which occurred on 6/26/2014. Most of the free product had been flushed into a containment pit and pumped out by a water truck. Some free product is still surfacing around the affected area, and oil absorbent pads are being used to continually gather it. Both 100 bbl tanks have been removed from the location, and the mechanical separators and drip will be removed this week. A track hoe is on location and is being used to scrape off contaminated soil. Oil absorbent booms are still being used in the drainage ditch to catch any oil that may migrate off location. The clean up is expected to be finished by the end of this week.

+ | | Upon inspection, it was observed that cleanup efforts at this location are complete. All old equipment that sat in the former tank battery has been removed, and new lines have been plumbed in with a domestic meter and new sales line. All contaminated soil has been excavated and hauled off to Athens-Hocking Reclamation Landfill. I have been provided with landfill load documentation by Mr. John Schneider of Stonebridge Operating. The location has been brought back to grade and seeded/mulched in an attempt to grow a new vegetative cover. This satisfies the requirements of the compliance notice.

| Performed final check on this location to ensure that vegetation was established over the remediated spill area. There is grass growing over the entirety of the remediated area. No oil has shown up at the surface, indicating that all of the contaminated soil was removed. New ID's are posted at the well and on the new plumbing setup. Well is in compliance. | | Performed final check on this location to ensure that vegetation was established over the remediated spill area. | Stopped in to check on status of cleanup at the tank battery for this well. Stonebridge Operating Co. roust crew was on location and was tearing down the mechanical separators and drip tanks for removal. New domestic and sales lines will be run in the next few days so the well can be put back in line. All of the contaminated soil has been excavated and loaded into a dump truck to be hauled away to Athens/Hocking Sanitary Landfill in Nelsonville. Soil on the location will be monitored for another week or so to ensure no more oil surfaces. Reclamation will be done on entire site. | | Stopped in to check on status of cleanup at the tank battery for this well. Roust crew was not present upon my arrival, however, it was evident that work had been done there this day. Oil absorbent pads are still in place in containment ditches and pit that was dug to flush free product into. Will continue to monitor this location until cleanup is complete. | ||

| Stonebridge Well Tender Mr. Randy Cisler reported a spill at this well at 10:04 p.m. on 6/25/2014. Between 4-5 bbl of oil was released from a 100 bbl production tank that was very corroded and developed a significant leak. Stonebridge's roust crew was on location and had contained all but approximately 0.5 bbl of oil that escaped into the drainage ditch during last night's heavy rain event. Oil absorbent booms were in place in spots along the drainage ditch and a tributary creek to the Little Muskingum River. The oil covered an area approximately 10' wide by 60' long. There was no dike around the tank battery to hold back a spill. No oil is believed to have made it into waters of the state. I witnessed Stonebridge's water truck pumping down the (2) 100 bbl tanks to empty. OEPA Environmental Specialist Mr. Trevor Irwin was on location when I arrived. I discussed a cleanup strategy with Mr. Jeremy Cassidy of Stonebridge Operating and he stated they will flush all of the free product into one area using a sump pump and hose, and then will use the water truck to pump off all of the free product on the surface. Mr. Cassidy will also bring in a skid-steer to scrape off all of the contaminated soil and it will be hauled off to an EPA approved landfill. Mr. Cassidy stated that they plan to have 90% of the cleanup effort completed today, and they will have it finished this weekend. The tank battery will be eliminated, as the wells using them are swab wells only and the tank battery is unnecessary. The wells that used these tanks will now be swabbed into portable poly tanks. | | Stonebridge Well Tender Mr. Randy Cisler reported a spill at this well at 10:04 p.m. on 6/25/2014.
