| I met the landowner Jim Flynn out at his property to inspect three wells. This well is equipped with a pumpjack with it head disconnected and laying on the ground. It was still attached to the rods. There is contamination from the stuffing box all around the wellhead. The valves were open and the landowner stated Andy Corp was out there in October repairing the leaking stuffing box. The flow line and electric line run through the creek down below and both are exposed in the water and in danger of breaking from debris. I talked with the field supervisor Jody Jasper and he said they would get on all of these problems this week. Operator must remove and properly dispose of all contamination and repair or replace any faulty equipment.

Yesterday they had removed all of the material from inside the dike and hauled it off location and had begun spreading new limestone gravel in and outside of the dike. The field crew also spread Black River High Calcium Hydrated Lime on all of the areas outside of the dike. Jody Jasper stated they were going to try and correct the line across the stream today as well.

There are several inches of snow on the ground therefore it makes it difficult to review the area outside of the dike. I will continue to monitor this location.

| Upon inspection, I found that the 1500 gallon tank was full and leaking in several areas. There were signs that the dike was holding brine. There is evidence of brine outside of the dike around the drain pipe. Quantabs revealed over 600 ppm of chlorides all around the outside and inside of the dike. I called and left messages with field supervisor, Jody Jasper, and Manager Rusty Hutson. I talked to well tender, Andy Corp and explained the issues. Operator needs to remove leaking tank and all contamination in and outside of the dike.
