| Joe Altier has asked me to come out to the tank battery to witness that he has replaced the leaking nipple. I arrived on location and found that he has in fact replaced the nipple and replumbed the lines to the plastic tank underneath the knock out tank. There are no other issues at this time.

| Mrs. Hivnor called and stated the nipple on the tank was leaking brine. When I arrived I found that there was a very small leak coming from the nipple which already had a clamp. Josh Garland was recently at location for the same complaint and had issued a compliance action. I told Joe Altier that they needed to repair or replace the equipment and clean up the contamination. I witnessed the well tender tighten the clamp and clean up the area. Joe said if the nipple still leaks he will replace the part. I will return in a few weeks for a follow-up inspection.

It had just rained so it was hard to tell if it was from the tank. I called Joe and he stated they have shut in well and recorded the levels and found no loss in the fluid. Joe does not believe the tank is leaking and we will have to wait until there is a long period of dry weather in order to tell if there is an issue.

| My inspection this day with Joe Altier found area with brine and oil kill has been reclaimed - contaminated soil removed, Joe says - smoothed ground seeded, fertilized, and mulched - good rain the last day or 2 and should be sprouting very soon.  Joe has replaced the steel flowline with plastic and corrosion inhibitor couplings and this should prevent any further problems.  The dike area was inspected and, due to rains the night before, was found to contain fresh rain water - was no indication of brine and Joe and I discussed the possibility that "the hose could have popped out of the hole in the top" and released brine inside the dike area.  The hose is now tied down and can't possiby slip out.  Joe opened valve on outside of dike and drained the inside of the fresh water.  Identification is complete.  All violations have been resolved.

| I received a complaint from Debbie Hivnor that grass has not grown back where Joey had reseeded and that there were still bare patches.  I went out and looked with Joey Altier and found there were a couple 2' squares of ground that still did not have a sufficient stand of grass - the big majority of it did.  Considering that he only reseeded about 3 weeks ago and that we did not have rain until the last week, I felt it looked very good.  I know that if another month passed, all the ground would be covered.  Joe said he would go ahead and put more seed down in those patches and also address another area where she felt was too bare.  This 2nd spot is where her cows root around and is understandable that grass has trouble growing.  Joe said he would also plant more there.  I called Debbie and told her that while I did not see a problem with the reclamation the way it was and that if she had waited a month, it would be taken care of,  that Joe was going to re-seed the areas for her.

The tank battery was equipped with two 100 barrel (Bbl) steel tanks, a 30 Bbl water knock-out not in use, a separator, and a 100 gallon poly tank being used for the annular disposal on another well on the lease. This well is not set up for annular disposal. The nipple on  one of the 100 Bbl tanks had corroded and allowed a small amount of salt water to leach into the dike. There was a band clamp temporarily wrapped around the nipple. The dike was equipped with a drain to release fresh water. There was no sign of contaminated soils near the end of the drain outside of the dike. The water in the dike was field tested with a Quan-tab and found to have greater than 6508 ppm Chloride content. All leaking equipment shall be repaired or replaced. Any contaminated soils shall be removed, and disposed of properly.

He said he would get a new tank and remove the contamination.

Altier and requested he meet me at site and discuss plans for possibly removing tank and replacing it with a new tank and also replacing stone that is located beneath present tank,decision was made to replace tank and stone at site owner stated job would be complete by 5/2/08.Photo's taken of site 4.

| I received call from Debbie Hivnor at 6:30 AM on 7/30 that Joe had released brine inside the dike area - said they had called him after discovering it at 10 PM - he said he would be out in morning.  Also stated that there was still an oily spot down in woods from his last "oil spill".  I called Joe and he said they had been out and flushed brine with clean water and removed from inside dike.  Very little - less than a bbl - had spilled out of tank.  Joe said he had a trac-hoe coming on Wed. to finish laying the new flowline that he continues to have leaks in.  He had received information for couplers that stop the corrosion that is causing his leaks.  Joe plans to have line replaced and restoration done by the end of the week.  He plans to removed contaminated soil, seed, mulch and fertilize the area in question and remove all brine from inside the dike.  No violation wrote at this time but instructed Joe what to do.

-1262785125 2012-10-10 My inspection this day with Jake Glascock to show him well, inform him of the numerous complaints he may receive. I gave Jake a history of the problems and we did a visual inspection of the recent reclamation, finding it satisfactory. The tank battery looked satisfactory and found no violations.
1325071346 2013-08-06 Debbie Hivnor called and said she believes the tank is leaking from the bottom. She said they have been digging away the gravel from under the nipple and it accumulates water. It had just rained so it was hard to tell if it was from the tank. I called Joe and he stated they have shut in well and recorded the levels and found no loss in the fluid. Joe does not believe the tank is leaking and we will have to wait until there is a long period of dry weather in order to tell if there is an issue.
1344366223 2012-08-06 My inspection this day with Joe Altier found area with brine and oil kill has been reclaimed - contaminated soil removed, Joe says - smoothed ground seeded, fertilized, and mulched - good rain the last day or 2 and should be sprouting very soon. Joe has replaced the steel flowline with plastic and corrosion inhibitor couplings and this should prevent any further problems. The dike area was inspected and, due to rains the night before, was found to contain fresh rain water - was no indication of brine and Joe and I discussed the possibility that the hose could have popped out of the hole in the top and released brine inside the dike area. The hose is now tied down and can't possiby slip out. Joe opened valve on outside of dike and drained the inside of the fresh water. Identification is complete. All violations have been resolved.
1346358722 2012-08-30 I received a complaint from Debbie Hivnor that grass has not grown back where Joey had reseeded and that there were still bare patches. I went out and looked with Joey Altier and found there were a couple 2' squares of ground that still did not have a sufficient stand of grass - the big majority of it did. Considering that he only reseeded about 3 weeks ago and that we did not have rain until the last week, I felt it looked very good. I know that if another month passed, all the ground would be covered. Joe said he would go ahead and put more seed down in those patches and also address another area where she felt was too bare. This 2nd spot is where her cows root around and is understandable that grass has trouble growing. Joe said he would also plant more there. I called Debbie and told her that while I did not see a problem with the reclamation the way it was and that if she had waited a month, it would be taken care of, that Joe was going to re-seed the areas for her.
1367765004 2013-05-05 Well equipped with a pumping unit with electric motor. The stuffing box had developed a leak and released a small amount of crude oil at the well head. The tank battery was equipped with two 100 barrel (Bbl) steel tanks, a 30 Bbl water knock-out not in use, a separator, and a 100 gallon poly tank being used for the annular disposal on another well on the lease. This well is not set up for annular disposal. The nipple on one of the 100 Bbl tanks had corroded and allowed a small amount of salt water to leach into the dike. There was a band clamp temporarily wrapped around the nipple. The dike was equipped with a drain to release fresh water. There was no sign of contaminated soils near the end of the drain outside of the dike. The water in the dike was field tested with a Quan-tab and found to have greater than 6508 ppm Chloride content. All leaking equipment shall be repaired or replaced. Any contaminated soils shall be removed, and disposed of properly.
1369842852 2013-05-29 The stuffing box has been repaired and the operator cleaned up the oil contamination. Well is in compliance.
1377539066 2013-08-26 Alan Hivnor called and stated that the tank was leaking and he was concerned about his spring water. He said there was standing water in the dike. Upon inspection, I found a puddle with around a quart or less of brine water which appeared to be emanating from the bottom of the tank. I called Joe Altier and told him the only way to know for sure whether or not the brine source is from the tank or trapped in the dike gravel is to remove both. He said he would get a new tank and remove the contamination.
1378322624 2013-09-04 Joe has cahnged out the tank, removed the old gravel, and replaced with new gravel. There are no issues at this time.
1381801607 2008-05-01 Complaint received from Allen Hivnor @10:30 pm of oil within dike area at tank site located on his property, he claims that tank is leaking only when owner heats oil in preparation for shipping, inspected site and found small amount of oil residue located within dike area called well owner J.V. Altier and requested he meet me at site and discuss plans for possibly removing tank and replacing it with a new tank and also replacing stone that is located beneath present tank,decision was made to replace tank and stone at site owner stated job would be complete by 5/2/08.Photo's taken of site 4.
-151702270 2013-07-03 Joe Altier has asked me to come out to the tank battery to witness that he has replaced the leaking nipple. I arrived on location and found that he has in fact replaced the nipple and replumbed the lines to the plastic tank underneath the knock out tank. There are no other issues at this time.

-2131216568 2013-06-12 Mrs. Hivnor called and stated the nipple on the tank was leaking brine. When I arrived I found that there was a very small leak coming from the nipple which already had a clamp. Josh Garland was recently at location for the same complaint and had issued a compliance action. I told Joe Altier that they needed to repair or replace the equipment and clean up the contamination. I witnessed the well tender tighten the clamp and clean up the area. Joe said if the nipple still leaks he will replace the part. I will return in a few weeks for a follow-up inspection.
-2131253285 2004-03-02 Complaint called in by Deb HIV nor stated there was oil/brine within dike area wanted it cleaned up A.SA.P.arrived on location to meet with well owner Joe Artier to witness cleanup, dike area was flushed with fresh water and loaded in vacuum truck owned by Steve AltierUIC-154photos taken of before and after cleanup.Mr. Hivinor called @8:40 pm and stated approximatly 1qt. Of iol still remained within dike area wants it cleaned up .3-3-04 went by site and took additional photo's of clean up.
-223977329 2012-10-10 I accompanied Cindy VanDyke on the inspection of well. Well is capable of producing and there is a current chart in the meter. There were 2//100 barrel tanks and a 25 barrel separator above a plastic water tank. ID was posted on tank and dike is in good condition.
