| I was on location with Electrical Safety Inspector Larry Elasivich (Ohio Dept. of Commerce) and Speelman Electric to accompany Larry during his inspection of the 600 amp, 240 volt electrical service that is installed on this pad.  Larry inspected the equipment and approved the power to be turned on.  He will contact AEP to let them know that the power can be turned on.  No violations.

| I was on location with North Region Supervisor Tom Hill to accompany specialists from the OEPA Division of Air Pollution Control so they could monitor emissions created by these producing operations.  There are 3 wells on this pad that have been recently completed and Rex Energy is currently conducting flow-back operations.  They are flowing the wells back through temporary separation units, flaring the gas through a large stack, and collecting fluids in steel frac tanks.  No violations.

| I was on location to observe the running and cementing of the 13.375" casing. Siderwinder Rig # 63 drilled a 17.5" hole to 1087'. Frank's Casing Crew ran the 13.375" casing to 1001'. Universal Well Services mixed and displaced the cement slurry back to surface. A total of 791 sacks of Type 1, 3% CaCl, 0.2% Foam Check- P,  .25 lbs. per sack celloflake comprised the cement slurry. Displacement went 2.5 barrels over the calculated amount at which time the well was shut in to allow the cement to cure. Water temp. was 53 F. , pH was @ 7.0. I observed  35 barrels of cement returned to the surface. No violations were identified at this inspection.
