0074794111 2017-06-19 Production Wells No Violations I conducted a status check of the site while HAD drilling was setting conductor pipe on the #1H, 2H, & 7H wells. I reviewed the site location according to the simultaneous operations plan submitted by Rex Energy. Sidewinder #62 was anticipated to move on site during the week of 06/12/2017. Cement barricades were in place as per the simultaneous operations plan to isolate all production equipment from drilling operations. I spoke to Andrew Campolongo, field coordinator for Rex, who informed me that the three western condensate tanks had been emptied as per the simultaneous operations plan. No violations were noted at the time of inspection.

Proper identification with the current operator was posted at the time of inspection. No violations were observed at the time of inspection. The compliance notice can be closed.
0203267231 2017-07-17 Production Wells No Violations I conducted a status check of the well while on location to witness the #7H surface cement job. The well is in production, but I did not locate a surface-intermediate annular pressure gauge after my request to Rex Energy to install one. I spoke to Larry Barr, production operator on site and asked a gauge to be installed. He informed me that he will install a gauge by 07/19/2017
0565454976 2017-05-17 Production Wells No Violations I was on site with DOGRM District Supervisor Stephen Ochs and Emergency Operations and Response Manager Terry Whitehair to meet with R.E Gas Development personnel to discuss a proposed SIMOPS plan. R.E Gas Development proposed drilling three new wells (7H, 2H, and 1H) on the pad while simultaneously producing the #3H, 4H, and 5H wells. We perfumed a walk through of the site with personnel and discussed any possible concerns with the pad with R.E Gas Development personnel who will submit a plan for DOGRM review.

No changes have been made since my last inspection. I did not locate a surface pressure gauge on the well head, but I contacted Shawn McGuigan of R.E Gas Development to have a gauge installed on the well head on 06/21/2017.
0813740662 2017-07-19 Production Wells No Violations I conducted a status check of the well. The well is in production and Sidewinder #62 was on site drilling the #1H well. A surface-intermediate annular gauge had been installed on the well and the gauge displayed zero psi. No violations were noted at the time of inspection.
0915041646 2017-05-17 Production Wells No Violations I conducted a status check of this well. I did not conduct a preliminary restoration at this time because R.E Gas and Development will be three more wells on the pad during 2017. Identification signs were posted at the access road entrance, the well, and the storage tanks. The #3H well is in production. The well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consists of the following: 9 - 400 bbl production tanks, three labelled as containing produced water, and six labelled as containing condensate. 1 Low pressure vertical separator 3 Producing wells 3 Sand separators 3 Gas processing units 1 Heat Treater 1 Vapor Recovery Unit

No violations were noted at the time of inspection.

I conducted a status check of the well and production pad. Six wells are located on the pad (permit numbers (22734, 22735, 22736, 22452, 224251, 22450). This well is equipped with plunger-lift configuration. The surface-intermediate annulus was equipped pressure relief valve and pressure gauge which displayed a reading of zero psi. Identification was posted at the well and lease entrance. The production tanks and production equipment were surrounded by a lined steel dike. The access road was stable and drivable. Erosional controls appeared to be functioning. Equipment on the pad consisted of the following: Nine 400 bbl production tanks; six labelled as containing condensate and four as containing produced water Six gas processing units with line heaters and vertical separators One combustor unit One vapor recovery unit One oil stabilization unit

1150949636 2014-07-28 Administrative Inspection No Violations I was on location with Electrical Safety Inspector Larry Elasivich (Ohio Dept. of Commerce) and Speelman Electric to accompany Larry during his inspection of the 600 amp, 240 volt electrical service that is installed on this pad. Larry inspected the equipment and approved the power to be turned on. He will contact AEP to let them know that the power can be turned on. No violations.
1364813484 2014-10-03 Completion Testing No Violations I was on location with North Region Supervisor Tom Hill to accompany specialists from the OEPA Division of Air Pollution Control so they could monitor emissions created by these producing operations. There are 3 wells on this pad that have been recently completed and Rex Energy is currently conducting flow-back operations. They are flowing the wells back through temporary separation units, flaring the gas through a large stack, and collecting fluids in steel frac tanks. No violations.
1397751819 2014-04-16 Casing String Witnessed No Violations I was on location to observe the running and cementing of the 13.375 casing. Siderwinder Rig # 63 drilled a 17.5 hole to 1087. Franks Casing Crew ran the 13.375 casing to 1001. Universal Well Services mixed and displaced the cement slurry back to surface. A total of 791 sacks of Type 1, 3% CaCl, 0.2% Foam Check- P, .25 lbs. per sack celloflake comprised the cement slurry. Displacement went 2.5 barrels over the calculated amount at which time the well was shut in to allow the cement to cure. Water temp. was 53 F. , pH was @ 7.0. I observed 35 barrels of cement returned to the surface. No violations were identified at this inspection.
1422288926 2015-01-15 Production Wells No Violations I was on location to conduct a status check of this well. The well is open flow with a wellhead tree assembly. Jersey barriers are installed around the wellhead. The tank battery consists of 9, 400 BBL production tanks and was diked. A windsock was installed on the top of the production tanks. The production equipment was also diked. Lightening protection was installed on poles around the production equipment. At the time of the inspection, the fresh water impoundment was still in place and located west of the site. The well is properly identified with a clear, legibly marked well identification sign. The lease road is stable and drivable. No violations found.

1440346025 2017-06-08 Production Wells No Violations I was on site to check the status of pad modification in anticipation of drilling three more wells on the pad. The pad had been widened to the west as per the plan submitted to DOGRM by R.E Gas Development. and was lined with gravel. No drilling equipment was yet on site. No violations were noted at the time of inspection.
1498089237 2017-06-19 Production Wells No Violations I conducted a status check of the well while on location for the surface casing cement job on the Jenkins #7H well. The well is in production. Barricades were installed around the production area, with additional barricades around the well head. I did not locate a surface annular pressure gauge on the well. I will contact Rex Energy to have one installed.

The surface/intermediate annular gauge displayed a reading of zero psi. Identification posted at the access and well did not list the current owner of PennEnergy Resources. Records indicate that ownership of the well was transferred from R.E. Gas Development LLC to PennEnergy Resources on 10/03/2018. Per OAC: 9-9-05, any changes of ownership shall be shown on posted identification no later than sixty days after the date of transfer. The owner shall posted current identification to well and tank battery by 01/14/2019.

I performed a walk-around inspection of the pad and erosional controls appeared in place and functioning. The well was equipped with an open-flow assembly.
