| I witnessed the plugging and abandonment of this well with Mr. Kip Thompson today.  As previously noted on 5/17/12, the well owner had placed 1.25" plastic inside the 3.5" casing for reverse circulation purposes.  A steel fitting had been attached to the bottom of this plastic line to keep straight.  Although minimal cement had been placed in the wellbore prior to pulling, the plastic line would not come free.  Prior to all activity, the well owner, Mr. Steve Altier, had requested and assured the Division this method would work instead of bringing in a service rig and running steel tubing to cement to surface.  On 7/12/12, Mr. Altier called and stated that he retrieved approx 450' of 500' plastic line with a dynamite cap, which had been placed inside the plastic line.  Mr. Altier was instructed that no notification was received.  On 7/13/12, the well was filled solid from total depth to surface with approx 1 yard of 9 sack grout inside 3.5" casing.  Note: When this was drilled, the 3.5" casing (annulus) had been cemented to the surface therefore, no casing was retrieved.  Instructed company representatives to leave well bore open over the weekend to allow for potential settling and check for leakage.  The previously noted violation(s) for non-production have now been resolved.
