0649378881 2012-07-13 Plug / Plug Back No Violations I witnessed the plugging and abandonment of this well with Mr. Kip Thompson today. As previously noted on 5/17/12, the well owner had placed 1.25 plastic inside the 3.5 casing for reverse circulation purposes. A steel fitting had been attached to the bottom of this plastic line to keep straight. Although minimal cement had been placed in the wellbore prior to pulling, the plastic line would not come free. Prior to all activity, the well owner, Mr. Steve Altier, had requested and assured the Division this method would work instead of bringing in a service rig and running steel tubing to cement to surface. On 7/12/12, Mr. Altier called and stated that he retrieved approx 450 of 500 plastic line with a dynamite cap, which had been placed inside the plastic line. Mr. Altier was instructed that no notification was received. On 7/13/12, the well was filled solid from total depth to surface with approx 1 yard of 9 sack grout inside 3.5 casing. Note: When this was drilled, the 3.5 casing (annulus) had been cemented to the surface therefore, no casing was retrieved. Instructed company representatives to leave well bore open over the weekend to allow for potential settling and check for leakage. The previously noted violation(s) for non-production have now been resolved.

0994159172 2011-02-15 Production Wells No Violations A follow-up inspection conducted with Mr. Jeff Fry, West Region Oil & Gas Supervisor, revealed non-compliance with Chiefs Order. The well consisted of 3 casing with a swage, valve and tied into a drip located next to the wellbore. The valve was observed closed. There were no observed gas sales, house gas, tanks or pumping unit. Identification was not displayed at the well. A new GPS reading was obstained for the well and compared to original plat map. The coordinates were sent to the Central Office for verification.
1236674677 2014-06-03 Final Restoration No Violations There are beans planted in the field where the well was located. There is no evidence of production equipment left behind and the land was properly restored. The location has met all 1509.072B restoration requirements.
1400332931 2012-05-17 Plug / Plug Back No Violations An inspection of plugging operations revealed well owners crew were unable to pull 1 plastic tubing from the wellbore. While adding a 9 sack grout mix, the well owners crew attempted to pull this tubing with no success. At this time, gas was observed inside the 1 tubing. The well owner (not on location) was instructed to correct deficiency. Note: Also advised no plug permit under this well and to file for permit; or notify Central Office if mistakenly filed under wrong permit number.
