| On 1/22/16, I performed an incident inspection at the Gulick site in response to a spill report received that morning and made the following observations:

•Per the operator a hose disconnected/was not connected properly while transferring reuse water between from a truck to a storage tank allowing roughly 50 gallons to spill out of containment and onto ice on the pad.  Of the 50 gallons spilled roughly half was reported to have been immediately removed via vacuum truck before the ice was melted.  The remaining amount was to be removed once the pad thawed (pad surface was hard frozen at the time of inspection).
