| Pre-operation inspection at the request of the Operator.  At the time of inspection the Erosion and Sedimentation plan was onsite and all BMPs appear to implemented and maintained as outline in the E&S plan.  The drill pit located onsite appears to be in sound condition with the required 2 feet of freeboard.  The Department notes that at the time of inspection an erosion ditch is forming  on the fill side of the well pad area and left unchecked could result in sedimentation transported offsite.  The Department suggests the Operator clean out the area behind the silt fence and mend the eroded fill side of the well pad.  The Department suggest the Operator continue to monitor the site weekly and after significant precipitation events to ensure site stability and BMPs are maintained.

| At the time of inspection the Operator was actively drilling the onsite.  The Erosion and Sedimentation plan was onsite and all BMPs appear to implemented and maintained as outline in the E&S plan.  The drill pit located onsite appears to be in sound condition with the required 2 feet of freeboard.  The Department notes that suggestions made during the 9/13 inspection in relation to the erosion ditch and sediment fence maintenance have not been corrected.  The Department suggests again that the Operator mend the erosion ditch  forming on the corner of the well pad furthest  from the cut slope and pull the built up sediment away from the sediment fence.  The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure site stability and BMPs are maintained.

Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2099870 Primary Facility 09/13/2012 Routine/Complete Inspection No Violations Noted Pre-operation inspection at the request of the Operator.
2104747 Primary Facility 10/03/2012 Routine/Complete Inspection No Violations Noted At the time of inspection the Operator was actively drilling the onsite.
2111180 Primary Facility 10/24/2012 Routine/Complete Inspection No Violations Noted At the time of inspection the site appeared to be drilled but not equipped for production The Erosion and Sedimentation plan was onsite and all BMPs appear to implemented and maintained as outline in the E&S plan. The drill pit located onsite appears to be in sound condition with the required 2 feet of freeboard. The Department notes that suggestions made during the 9/13 and 10/3 inspection in relation to the erosion ditch and sediment fence maintenance have been corrected. The Department notes that the Operator mended the erosion ditch forming on the corner of the well pad furthest from the cut slope and pulled the built up sediment away from the sediment fence. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure site stability and BMPs are maintained.
2118997 Primary Facility 11/28/2012 Drilling/Alteration No Violations Noted Well producing in line after recent frac. Pit lined and freeboard maintained.
2170517 Primary Facility 05/24/2013 Site Restoration No Violations Noted Well appeared to be equipped for production, brine tank located onsite. Brine tank appeared to be in sound condition. Site had been regraded, seeded, mulched and the vegetation ahd begun to emerge. The operator should continue to monitor the site to isnure the continued growth of the vegetation. There was no observed accelerated erosion or sedimentation observed in relation to the site or access road.
2185962 Primary Facility 07/19/2013 Site Restoration No Violations Noted Well appeared to be equipped for production, brine tank located onsite. Brine tank appeared to be in sound condition. Site had been regraded, seeded, mulched and the vegetation met the 70% uniform coverage criteria. There was no observed accelerated erosion or sedimentation observed in relation to the site or access road.
2381108 Primary Facility 06/22/2015 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production, brine tanks located onsite and they appeared to be in sound condition. There was no observed accelerated erosion or sedimentation in relation to the site or access road.
2750679 Primary Facility 2018-07-11 Routine/Complete Inspection No Violations Noted This well is tagged and in production. The annulus is open. The well head casing, piping, production set equipment and two grounded 100 bbl fiberglass fluid collection tanks look ok.
