2497476 Primary Facility 2016-05-27 Administrative/File Review No Violations Noted 5/27/16: The DEP has reviewed the 5/2/16 document titled "Site Characterization Summary” for the Signor Well Pad 1H release reported to the DEP on 3/20/16 by Chief O&G. The report was reviewed in accordance with Alternative Remediation provision of department policy document 800-5000-001 Addressing Spills and Releases at Oil and Gas Well Sites or Access Roads. No technical deficiencies were noted.
2529568 Primary Facility 2016-10-20 Routine/Complete Inspection No Violations Noted Routine inspection to assess site conditions during flowback operations.

Screened electrical conductivity in puddls on the well pad surface. Readings ranged from 242 uS/cm to 879 uS/cm. No evidence of any spills such as staining, sheens, or odors were observed.

An area of erosion has started near the southeast corner of the well pad. This appears to be the low corner of the pad, and it appears the flow has concentrated and eroded pad material down the fill slope. None of the eroded material has left the limit-of-disturbance for the site. Merle Wade of Chief was on site and informed of this finding.

No violations were cited during this inspection.

2534935 Primary Facility 2016-11-02 Follow-up Inspection No Violations Noted Arrived @ 11:17. Routine inspection of unconventional gas pad following stimulation and flow back. The site was wet following heavy rain, a worker onsite was vaccing up standing water. Flow back equipment was onsite. The Signore North Unit 4H and Signore North Unit 5H were in production. The 1H was not in production at the time of my inspection. Left location @ 11:30.
2537579 Primary Facility 2016-11-17 Complaint Inspection Viol(s) Noted & Immediately Corrected Inspection conducted in response to spill reported to the Department by Chief Oil & Gas, LLC (Spill Notification ID No. 323649). Chief reported that approximately 20 gallons of brine leaked during the replacement of a produced water tank to the ground surface. The spilled fluid was immediately removed with a vacuum truck and the affected pad substrate was excavated and removed. New gravel was put in place.

Mitigation efforts had been completed at the time of my inspection. New gravel was evident at the location of the spill. No odor or staining that could indicate residual contamination was noted.

Because the brine spilled on the pad surface, this release had the potential to pollute waters of the Commonwealth, which is in violation of 35 P.S. § 691.402(b). Title 35 P.S. § 691.402(b) prohibits conducting activities regulated by permits issued pursuant to Section 402 of The Clean Streams Law to prevent the potential of pollution contrary to the issued permits or without a permit. 2812127 Primary Facility 2018-12-06 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 12:57; the weather was ~30°F and windy.

There were 3 properly tagged Marcellus wells staged for production. The well cellars were filled with ice. There were 3 GPUs onsite and in secondary containment. The secondary containment onsite was about 25% filled with ice.

The pad area was reduced in size and the site was greater than 70% vegetative cover. No E&S issues were observed.
