The vegetation is good and binding. There were no issues at well. There will need to be a bull plug on the piping at the valves below the poly tank. Supervisor called Company and explained what needed done. He was instructed to call Patrick Shreve on the completion of the installation of the plug.

| Well is supplied with pumping unit and tubing head. The tubing annulus and the rod annulus are open. The packing was leaking at the polish rod and had sprayed crude around the pumping unit. There is 100 barrel tank and a 30 barrel poly tank. They are connected at the bottom. There is a dike and identification in place. There is a 2line coming off of the bottom of the tanks and is laying over the dike. There is a large scald from end of 2line that is approximately 80-100 down over the hill and is 20-30wide. Then it hits a rock trough. From there it approximately 150down over hill again until it drops straight down over high wall and into strip mine pond that is full of fresh water. The strip mine pond approximately measures .25 to .5 acre. I took field samples to verify where the brine was in the soil. Field Sample 1 or (FS #1) had cords at 39.84015,-82.13953. This was end of dump line and the chlorides were above 6300ppm. FS#2 was at cords 39.84019,-82.13944. This sample was approximately 75down slope. The chlorides were above 3600ppm. FS#3 cords were at 39.84026,-82.13941. The chlorides were over 2400ppm. FS#4 was at cords of 39.84006,-82.13918. The chlorides were above 5700 ppm.This sample was approximately 250-300 down slope. FS#5 was at cords 39.84081,-82.13965. This sample was taken 40north of tank battery. The sample had less than 31ppm of chlorides. There was a control sample taken at cords 39.84026,-82.13980.  The sample was taken at 33west of the tank battery. The sample had less than 31ppm of chlorides.

| I entered the well site and Darrel Porter and his contractor Andy Browning is loading up the equipment and pulling out. He failed to notify the inspector Patrick Shreve of starting the work. Work was initiated on 10-4-15. They bio-remediated over the scald. There was no soil removed. Then they enlarged the trough and the contractor had a small poly tank and flushed the soil and hauled the brine water to Porters house. The contractor was not a licensed UIC Hauler. I stated that hauling brine water is regulated under Underground Injection Control (UIC). I took pictures of the work they done. I took a field sample at the new trough made by Browning and tested the soil at the sump. The sample showed below 31 ppm.

| I stopped by site to see if company started the clean up. They cleaned up oil at the well head. They have fixed the packing at the stuffing box on the pumping unit. They did not start on any brine scald clean up below the tanks. I called company and they stated that they had till October and I told them that the company had 7 days from the 9-16-15 only. She stated that Darrel was on vacation and would be back on October 3rd, 2015. I stated to have Darrel call me when he gets back.

| Brent Bear and I met with Darrel Porter of Porter Petroleum. We explained the samples that were taken and the process of clean up. The soil is to be removed and was allowed to be bio-remediated on site. The soil would be from end of dump line to top of rock trough. The soil would be dug down to virgin soil and placed on a liner that is going to be liquid tight and incorporate lime to neutralize the soil. Then place a liner catch basin at where the FS #4 was and flush with fresh water to bring the chloride levels down. The rock trough is unstable and should not be disturbed. There was verbal Compliance Notice issued from this date of 9-16-15. The agreement between us was 1 full week. He was to notify me when he would be in on the location.

There is vegetation on the soils that had a scald from brine. The vegetation is good and binding. There were no issues at well. There will need to be a bull plug on the piping at the valves below the poly tank. Supervisor called Company and explained what needed done. He was instructed to call Patrick Shreve on the completion of the installation of the plug.

0797251066 2015-09-14 Production Wells Violations Noted Well is supplied with pumping unit and tubing head. The tubing annulus and the rod annulus are open. The packing was leaking at the polish rod and had sprayed crude around the pumping unit. There is 100 barrel tank and a 30 barrel poly tank. They are connected at the bottom. There is a dike and identification in place. There is a 2line coming off of the bottom of the tanks and is laying over the dike. There is a large scald from end of 2line that is approximately 80-100 down over the hill and is 20-30wide. Then it hits a rock trough. From there it approximately 150down over hill again until it drops straight down over high wall and into strip mine pond that is full of fresh water. The strip mine pond approximately measures .25 to .5 acre. I took field samples to verify where the brine was in the soil. Field Sample 1 or (FS #1) had cords at 39.84015,-82.13953. This was end of dump line and the chlorides were above 6300ppm. FS#2 was at cords 39.84019,-82.13944. This sample was approximately 75down slope. The chlorides were above 3600ppm. FS#3 cords were at 39.84026,-82.13941. The chlorides were over 2400ppm. FS#4 was at cords of 39.84006,-82.13918. The chlorides were above 5700 ppm.This sample was approximately 250-300 down slope. FS#5 was at cords 39.84081,-82.13965. This sample was taken 40north of tank battery. The sample had less than 31ppm of chlorides. There was a control sample taken at cords 39.84026,-82.13980. The sample was taken at 33west of the tank battery. The sample had less than 31ppm of chlorides.

1165410773 2015-10-05 Production Wells No Violations I entered the well site and Darrel Porter and his contractor Andy Browning is loading up the equipment and pulling out. He failed to notify the inspector Patrick Shreve of starting the work. Work was initiated on 10-4-15. They bio-remediated over the scald. There was no soil removed. Then they enlarged the trough and the contractor had a small poly tank and flushed the soil and hauled the brine water to Porters house. The contractor was not a licensed UIC Hauler. I stated that hauling brine water is regulated under Underground Injection Control (UIC). I took pictures of the work they done. I took a field sample at the new trough made by Browning and tested the soil at the sump. The sample showed below 31 ppm.
