| On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing equipped with a plunger lift system.  At the time of my inspection annular pressure monitoring was in place on the 9.625-inch diameter deep intermediate casing and pressure was at 235 pounds per square inch.  The 13.375-inch diameter casing was stubbed up to surface with a 2-inch diameter valve that was cracked at the time of my inspection.  Production equipment included a Gas Processing Unit, as well as 6- 400-barrel steel production fluid tanks, and a combustor that was shared with the other producing wells on the pad.  There was secondary containment around the production fluid tanks, and the Gas Processing Units.  The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location.  There was a Mark West midstream valve site near located on the northwest corner of the pad.  Identification tags were present, and production was up to date.

| On the day of inspection accompanied by ODNR Senior Health Physicist Paul Carder, The well was equipped with a production tree with valves in the open position and a gauge present read 305 pounds per square inch on the casing. Production equipment included a sand trap, a line heater and a GP Unit with a 4 inch sales meter with a 1.5 inch orifice and chart recorder. Also present a vapor tower and 6-400 barrel tanks all within containment and a gas combustor all shared with the Noble Unit 1H and Guernsey Unit 2H. A fenced in water impoundment with life preservers is located south west of the well pad.

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0048956068 2012-07-16 Drill / Deepen / Reopen No Violations Union rig #63 rigging up on site to drill stat test well they should have rig ready to drill by 7/19/2012.
0457033076 2019-01-07 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing equipped with a plunger lift system.
0975179392 2017-01-05 Production Wells No Violations On the day of inspection accompanied by ODNR Senior Health Physicist Paul Carder, The well was equipped with a production tree with valves in the open position and a gauge present read 305 pounds per square inch on the casing.
1212888866 2014-03-31 Production Wells No Violations My inspection today finds the well appears to capable of production. The tank battery consists of eighteen 400 bbl tanks. Identification is posted in accordance with 1501.9-9-05(A)10. The lease road is in good repair. Production records are current.
1407440118 2017-03-16 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Annular pressure monitoring was present. The production equipment consisted of three gas production units, six 400 barrel tanks, and one combustor. Identification was present and production records were current. Antero Resources was on location preforming maintenance on a gas production unit.
1502369580 2017-08-10 Production Wells No Violations The purpose of my inspection was a status check on a production well. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Annular pressure monitoring was present. The production equipment consisted of three gas production units, six 400 barrel tanks, and one combustor. Identification was present and production records were current. Construction equipment was parked on the east side of the pad.
1541356279 2013-02-21 Completion Testing No Violations My inspection today finds the frac plugs are being drilled out by Mercer Well Service. They are currently at 8220.
1791729647 2012-11-26 Completion Testing No Violations Stopped by drill site to witness frac job,Frac Tech was on site pumping job for stage seven and they plan on performing fifth teen stages total on said well.
