| I conducted a compliance notice follow-up inspection for compliance notice 213527810.  The well was equipped with a plunger lift assembly and the annulus and flow line valves were open.  A domestic gas supply was plumbed off of the wellhead.  No identification was posted at the well.  No fencing was installed around the well and was not waived during the pre-site inspection.  The diked tank battery has two storage tanks and a separator.  The pick-up line was secure with a plug and lock.  The required urban safety equipment still had not been installed per 1501:9-9-05(E)(4)(b-e).  There was an identification sign posted at the tank battery, but was still missing the required information per 1501:9-9-05(E)(1)(a-e).  There is a locked chain link fence surrounding the tank battery.  The 210 BBL tank extends more than 3 foot above the fencing.  The access was stable but no gate has been installed, at the entrance.  Install the required gate with a lock at the access road entrance per 1501:9-9--9-

The equipment on the site consists of the following:



On 12/13/2016, Northeast Regional Supervisor Tom Hill and I met with Justin Caldwell, co-owner of PAC Drilling, and Rusty Hornhill, well tender for PAC Drilling concerning the missing required urban safety equipment on 31 PAC Drilling wells in Hubbard and Liberty Townships. Mr. Caldwell has been given a document listing the wells with the missing the urban safety equipment. PAC Drilling is to submit a schedule for equipment installation for review and approval by the Division of Oil & Gas.

1527009504 2018-05-14 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 213527810. The well was equipped with a plunger lift assembly and the annulus and flow line valves were open. A domestic gas supply was plumbed off of the wellhead. No identification was posted at the well. No fencing was installed around the well and was not waived during the pre-site inspection. The diked tank battery has two storage tanks and a separator. The pick-up line was secure with a plug and lock. The required urban safety equipment still had not been installed per 1501:9-9-05(E)(4)(b-e). There was an identification sign posted at the tank battery, but was still missing the required information per 1501:9-9-05(E)(1)(a-e). There is a locked chain link fence surrounding the tank battery. The 210 BBL tank extends more than 3 foot above the fencing. The access was stable but no gate has been installed, at the entrance. Install the required gate with a lock at the access road entrance per 1501:9-9--9-

Identification still needs posted at the well. No fencing was installed around the well and was not waived during the pre-site inspection. The required urban safety equipment still had not been installed per 1501:9-9-05(E)(4)(b-e). There was an identification sign posted at the tank battery, but was still missing the required information per 1501:9-9-05(E)(1)(a-e). There is a locked chain link fence surrounding the tank battery. The 210 BBL tank extends more than 3 foot above the fencing. The access had a chain across it, but a locked gate is still required to be installed with proper identification. Install the required gate with a lock at the access road entrance per 1501:9-9-05E(2) and add the identification at the gate per 1501:9-9-05E(1)(f).

PAC needs to: 1) Correct the identification sign to be in compliance with 1501:9-9-05E(1)(a-e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 2) Raise the fencing around the tank battery or use low profile tanks to be in compliance with 1501:9-9-05E(4)(f). 3) Install the required gate with a lock at the access road entrance per 1501:9-9-05(E)(2)(c). 4) Post the proper identification sign on the access road gate per 1501:9-9-05E(1)(f). 5) Install the fencing around the well and install indentification. 6) Install the required urban safety equipment, spark arrestor, low-pressure relief valve and charcoal filter, per 1501:9-9-05(E)(4)(b-e).

1768971003 2012-09-05 UrbanProduction Wells No Violations I completed an urban production well inspection. My inspection found the well was equipped with a plunger lift unit (rabbit) that was in good condition. The tank battery was surrounded by a chain link fence and contained one 210 barrel oil barrel storage tank, one 50 barrel brine storage tank, a separator, and the proper identification posted. I noticed no visible spills or leaks. The lease road was in good condition. There are no violations at this time.
