| The Canal Place # 3-D well was completed 2/96. The well is located in the parking lot of the Spaghetti Warehouse. The wells production is current as reflected by RBDMS. The well is operated by an electric pump jack. The well construction is 8.625 surface casing ( 352 ), 4.5 production casing with 2.375 tubing. A downhole pump is actuated by the sucker rods.  The  signage at the well is complete and legible. The well is surrounded by a locked fence and evergreen landscaping. There are six ( 6) 55 gallon barrels marked non hazardous waste on the north side of the well. There was not any sign of a previous release around the well head. I will inquire with Operator as to the nature of the material and request its disposal. All the piping and fittings were free from leaks during my inspection. The tank battery is shared with the Canal Place # 1 well ( 34-153-2-2772 ). The tank battery is surrounded with a concrete dike with chain link fencing on top. The fencing was locked. The required urban production I.D. signage was in place and legible. The tank battery ( for the Canal Place # 3 ) consisted of one (1) 140 barrel oil tank, one (1) poly 25 barrel brine tank and a 2408 mechanical separator. The tank battery was free from leaks and contamination. There were no violations identified during my inspection.

| I conducted a status check inspection of this directional, Clinton Sandstone well prior to the commencement of plugging operations scheduled for next week beginning 8/1/2016.  My inspection found the wellhead equipped with rods and tubing.  The pumping unit has been removed and the area around the wellhead remains fenced and secured.  The valves on the tubing head and 4.5 production casing are open to the tank battery and the valve on the annulus between the 8.625 surface casing and 4.5 casing is closed.  The proper ID and warning signage is posted on the fencing.  The well produces to a tank battery to the south that is shared with the Canal Place #1 (PN#2772) and contains two (2) horizontal separators, two (2) 140 barrel tanks, and two (2) 35 barrel poly tanks.  The battery has been constructed 4-5 below the grade of the parking lot and is fenced, screened, and secured with locks.  The domestic hook-up for Canal Place has been disconnected.  The proper ID and required warning signage are posted on the tank battery fence.  No violations.

| The previous location for the Canal Place # 3-D has been paved over, with asphalt, and was being utilized as part of the parking lot for the Canal Place complex. The wellhead and tank battery were accessed on existing roads within the complex. The tank battery was completely removed and paved. I did not find any surface expression of previous oil and gas activity during this inspection. The flow lines were reported to be purged with fresh water. The Owners of Canal Place have submitted a Landowner Waiver to allow the flowlines to remain in place. The Canal Place # 3-D was in compliance to meet the Final Restorations parameters. Final Restoration was passed.

| I was on location at 6:50am with Nicks Well Plugging (NWP), Appalachian Well Surveys (AWS), and Ralph Gregory (Bass Energy) to witness plugging operations on this Clinton Sandstone well.  AWS shot off the 4.5 production casing at a depth of 2150 and NWP set a 300 plug to isolate the Big Lime and a 200 plug at he bottom of the surface casing.  AWS set a steel bridge plug at a depth of 210 and NWP filled the 8.625 surface casing with cement from the top of the bridge plug to ground level.  The hole was topped off and the casing will be cut off and fitted with the proper ID plate next week.  See plugging report for more details on this job.  No violations.

| I was on location at 9:30am with Nicks Well Plugging (NWP) and Ralph Gregory (Bass Energy) to witness plugging operations on this Clinton Sandstone well.  NWP recovered the tubing remaining in the hole and rigged up to bullhead the bottom plug.  NWP mixed and pumped 30 sacks (6.3bbls) of Class A cement with 50lbs of cottonseed hulls into the casing and bullheaded the cement plug with 63 barrels of water.  The plug landed and the well locked up at 600psi.  The well was shut-in and plugging operations will continue tomorrow (8/5/16).  No violations.
