At 5 minutes 660 psi, at 15 minutes 650 psi. A master valve was installed on the injection tubing and the valve is closed.

| Permit to Convert to Salt Water Injection Well 8-8-2018. Berry Well Service rig on site to clean out open hole portion of the well. Producers service corporation is on site to do an acid treatment. An earthen pit lined with plastic has been constructed next to the well along side of the service rig. The earthen pit is being used to circulate fluid from the well. There is a steel half round tank on a trailer on the opposite side of the service rig that is being used for fresh water storage. I told Tom Atha that the Earthen pit is a permit violation. The Permit to Convert dated 8-8-2018 states: FLOODPLAIN AREA: NO EARTHEN PITS ALLOWED. The permit also states: BLOW OUT PREVENTOR REQUIRED The service rig was attempting to clean out the open hole portion of the well with out well control device installed on the wellhead. The earthen pit is to be cleaned out and plastic liner removed. Backfill the pit. All fluids to be taken to an approved class 2 injection facility.

1129109188 2018-09-20 Production Wells No Violations Permit to Convert to Salt Water Injection Well 8-8-2018. Berry Well Service rig on site to install used 2 3/8 inch PVC lined steel tubing with a new Parmaco Shear Tension Packer. Packer set at 2509 feet. Producers pump truck circulated fresh water and inhibitor into MartinDill Vac truck. Packer was set and annulus was pressured up to 660 psi. At 5 minutes 660 psi, at 15 minutes 650 psi. A master valve was installed on the injection tubing and the valve is closed.

Installed 1650 feet of 2 inch fiberglass injection line from the well to the containment dike. The line is approximately 3 feet deep. The injection line is not connected to the injection tubing or the injection facility. The injection line was tested at 600 psi for 15 minutes with no loss in pressure.

The 2 inch fiberglass injection line was tested from the injection pump to the injection tubing master valve. Test was at 590 psi, at 5 minutes 590 psi, at 15 minutes at 585 psi. A Murphy switch is installed in line with the injection pump. The 300 barrel fiberglass tank in the containment dike is anchored with four cables. Identification is posted. The containment dike is free of fluids.
1725502983 2018-09-20 Production Wells No Violations Permit to Convert to Salt Water Injection Well 8-8-2018. Berry Well Service rig on site to clean out open hole portion of the well. Producers service corporation is on site to do an acid treatment. An earthen pit lined with plastic has been constructed next to the well along side of the service rig. The earthen pit is being used to circulate fluid from the well. There is a steel half round tank on a trailer on the opposite side of the service rig that is being used for fresh water storage. I told Tom Atha that the Earthen pit is a permit violation. The Permit to Convert dated 8-8-2018 states: FLOODPLAIN AREA: NO EARTHEN PITS ALLOWED. The permit also states: BLOW OUT PREVENTOR REQUIRED The service rig was attempting to clean out the open hole portion of the well with out well control device installed on the wellhead. The earthen pit is to be cleaned out and plastic liner removed. Backfill the pit. All fluids to be taken to an approved class 2 injection facility.

2052104282 2018-01-22 Drill / Deepen / Reopen No Violations Kenny Rich drilling has cleaned out the plugged well to 2595 feet. They ran 2552 feet of 4.5 inch diameter casing. Cemenco ran 4 centralizers on the 4.5 inch diameter casing and 75 sacks of neat cement with good returns to the pit. The packer will be set at a later date. The casing was tested at 600 psi for 5 minutes with no loss in pressure.
