2368776 Primary Facility 2015-05-11 Routine/Complete Inspection No Violations Noted Cabot and the township agreed to conduct dust control operations on the township road, Beaver Meadow Road, in front of the complainant’s. Cabot stated that they would handle dust suppression on their access road. The Department arrived on-site and observed the township applying calcium chloride to the township road. Dust suppression activities had not occurred on the access road yet. Dust was observed leaving the access road during the inspection. Left site. The Department contacted the complainant later in the day who stated that he was pleased with actions taken by all parties. No further action will be taken by the Department at this time.
2428324 Primary Facility 2015-11-24 Incident- Response to Accident or Event No Violations Noted Steve Watson, Environmental Health and Safety Specialist of Cabot Oil and Gas contacted me at 12:37 PM on 11/23/15 to report a spill of (68) gallons of production fluid that occurred around 10:30 PM on 11/22/15. Mr. Watson stated that sand washed out of a ball valve on a dump line due to mechanical failure releasing production fluid out of the J Reynen 6H Gas Production Unit onto the concrete containment for the GPU and eventually to the pad surface below the GPU. Apparently the failure of the valve immediately created a flash fire on this GPU. Mr. Watson claimed he was not made aware of the spill on 11/22/15. He investigated the incident during the morning hours on 11/23/15. A truck driver discovered the fire while on-site in the process of unloading production fluid from the production fluid tanks. The driver reported the fire to Cabot upon discovery. The GPU was significantly damaged from the fire. There is staining observed on the concrete containment and a small amount of fluid pooled on the backside of the GPU unit.

All the annulus vent pipes were connected to one pipe going into the same tank. There was 491psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 12:25p.m.

The complaint is that stormwater runoff coming off the site is impacting the complainant's pond. This inspection found that stormwater runoff from the site is not impacting the complainants pond. However, photos 1 thru 5 show that the entrance of the access road is overwhelmed by mud from the degradation of the access road due to the heavy truck traffic for the fracking operations. Sediment has buried silt sock, pushed silt sock, overtopped the silt sock, and gone underneath the silt sock. A plume of sediment was found off the ROW by approximately 50'. Photo 6 shows the cross culvert entrance prior to the tank pad access road buried under mud. Photo 7 shows the location of a small spill that occurred over the weekend on the tank pad. 2722545 Primary Facility 2018-04-20 Routine/Complete Inspection No Violations Noted On 4/19/18, Steve Catalfamo, Cabot EH&S Specialist contacted the Department after hours to report the release of hydraulic fluid to the tank pad of the Reynen J P1 Well Site. The spill occurred while a water truck was delivering fresh water and a hydraulic hose ruptured. At the time of the release, Cabot estimated that approximatey 20 gallons was released to the pad surface with approximately 5 gallons recovered immediately. Today's inspection was conducted with Steve Catalfamo. Resource Environmental was on-site during the inspection, and was conducting final soil sample tests to confirm that the remediation was successful. The Department finds that this spill has been immediately corrected. Next, photos 3 thru 5 show that the violations documented in inspection record # 2713352 have been corrected. The inspection also revealed that the violations related to the cross culvert located near the tank pad have also been corrected. 2744543 Primary Facility 2018-06-22 Routine/Complete Inspection No Violations Noted At time of inspection.

There were 7 wells on site, the 3H, 5H and 6H wells were existing wells and the 1H, 4H, 7H and 8H wells were recently completed and put in production. The wells tags for the existing wells weren't put back on the wells. The well tags on the 4H, 7H and 8H wells were correct. The well tag on the 1H had a permit number (115-21856) for a well that was reported never drilled, the correct permit number is (115-22357). I contacted Steve Watson of Cabot Oil & Gas to have the well tags for the three existing wells put back on and change the permit number on the 1H well tag. I also asked that Cabot send me a picture when the corrections are made. The 3H well was temporarily shut in. Ben Burridge (Cabot well tender) was flowing fluid off the 6H well because it was loaded up from the recent frac. The 5H well was back in production. 2754596 Primary Facility 2018-07-16 Incident- Response to Accident or Event No Violations Noted Frank Hoehle, DEP O&G WQS and I arrived at the site at 11:31 in response to a reported spill of (13) gallons of flowback fluid to the pad surface. The weather conditions are partly cloudy skies, hot and humid, with temperatures in the mid to upper 80's. Brody Webster and Steve Watson of Cabot Oil and Gas were at the site in the process of assessing the spill incident and overseeing the delineation of the extent of the impact. Resource Environmental is the subcontractor conducting field soil conductivity screening in order to delineate the extent of the contamination. GasSearch Drilling Services (GDS) is the subcontractor remediating the impacted area via an excavator. Mr. Watson and Mr. Webster provided the Department the following information in reference to the spill incident. "The spill was discovered on Saturday, 7/14/18, at 8:30 AM by a Cabot well tender. 2754872 Primary Facility 2018-07-18 Follow-up Inspection No Violations Noted The Department, Casey Baldwin, Frank Hoehle, DEP O&G WQS's and I arrived at the site at 13:23 to conduct a follow-up inspection. The weather conditions are partly cloudy skies with a light breeze and temperatures in the low to mid 70's. Brody Webster, Manager of Safety and Environment of Cabot Oil and Gas contacted me at 10:30 AM on 7/18/18 providing an updated status of the remediation and assessment of the spill incident. Mr. Webster stated the following during the phone conversation. "We discovered the area of the pad surface has been impacted where the gas buster tank and containment for the tank were formerly located, after removing the tank and containment. We are unable to quantify the volume of fluid spilled to the pad surface. Therefore, we will collect background and attainment samples consistent with Act 2 standards and seek Act 2 relief of liability."

2763205 Primary Facility 2018-08-09 Routine/Complete Inspection No Violations Noted I inspected the Reynen pad and access road. The wells associated with this pad were all properly labeled.

The cut and fill slopes were all well vegetated and stable at the time of inspection.

The access road has ben having runoff issues due to torrential rains but Cabot has been working with the Department and the landowner to address the roadside ditch outside the LOD.

There were no violations.

2770166 Primary Facility 2018-08-31 Routine/Complete Inspection No Violations Noted I inspected the Reynen pad as a follow up to a complaint received that the access road roadside ditch washed out and deposited sediment near a swale entrance and was causing water problems for a home downgradient of the road. The ditch was not within the LOD for the well pad however Cabot worked with the Department and the Landowner to repair the situation. The ditch was regraded and reworked following torrential downpours, and large stone lining the ditch was added. Accumulated sediment which had previously washed out a field entrance and filled a culvert pipe was removed and a larger culvert pipe was added. The field entrance was regraded and the swale was restored to working condition. The access road ditch is now in excellent condition.

The well pad surface had a plastic covering in an area of a previous release. The cut and fill slopes were well vegetated and the access road was regraded and stable.

There were no erosional issues apparent at the time of inspection.

2848940 Primary Facility 2019-03-06 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was empty and there was no gas detected. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was no gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 3/4/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
